Digital Care Coordinator

Job summary

The Digital Care Coordinator will support patients and colleagues in the use of digital healthcare technology to optimise access to the right person first time. They will also work to ensure all patients have an equal opportunity to access our services and working improve health inequalities through support for no digital access and an understanding of the needs of hard to access groups.

You will possess excellent communication skills both verbally while on the phone to patients as well as utilising written communication and templates for information gathering and reporting. You will be confident in using digital resources including Microsoft packages and the clinical systems which support our access and patient records.

Main duties of the job

The main duties will include:

  • Creating and updating dashboards to show the progress against local and national targets
  • Extracting and submitting data on a monthly or quarterly basis for payment
  • Improve patient access to digital resources, including the NHS App and practice website.
  • Increase understanding of issues faced by patients who are hard to reach, highly complex or digitally excluded and ensure patients are contacted in an appropriate method to meet their communication needs
  • Work in cooperation with the rest of the Digital Team to update and develop our clinical system (SystmOne) and other key digital tools used in primary care

Job responsibilities

Main duties of the role:

  • Processing new starter / leaver log ins in a timely manner
  • Compile and submitting data returns for payment in line with the strict monthly and quarterly deadlines.
  • Creating and maintaining Excel dashboards to monitor progress against local and national targets
  • Support the Operational Management and Digital transformation teams with communications to patients and assist in the processing of patient responses.
  • Increase understanding of issues faced by patients who are hard to reach, highly complex or digitally excluded and ensure patients are contacted in an appropriate method to meet their communication needs

  • Updating the Priory Medical Group intranet with regular news updates and key information relating to the organisation
