Job summary

Would you like to be part of the new Urgent Care Delivery 7-day service within the community setting in York?

If you are an experienced paramedic who is passionate about patient centred care, wanting to enhance your primary care clinical skills and develop your leadership potential then we are the urgent care provider of choice for you!.

You will work closely with a team of nurses, physiotherapist, occupational therapists, therapy assistants, Healthcare Support, GP, dietician, paramedics and social prescriber to support patients' individual needs, promoting patient health and wellbeing in their own homes.

Main duties of the job

The role primarily involves visiting patients in their homes who have been deemed as an urgent case, acting as an autonomous clinician to assess, treat and/or appropriately refer patients to specialist services.

You will work independently to assess and evaluate patients and ensure the continuity of care, including follow up.

Working within the York Integrated Community Team means you are part of a local, wider health team who support you as you progress in your career.

Job responsibilities

What are the main duties of the role?

  • YICT cover all 10 Practices in York and you will be working closely with Primary to offer an urgent care response in the community
  • You will be responsible for providing face to face visits patients who have been triaged and deemed to require a 2 hrs response.
  • You will act as an autonomous clinician and take responsibility for the assessment, treatment and/or to appropriately refer patients to specialist services when required
  • Be compliant with the NMC or HCPC Code of Professional Conduct and any associated national guidelines and abide by their guidance.
  • Take a comprehensive patient history, carry out physical examinations and use expert knowledge and clinical judgement to identify the potential diagnosis
  • Refer patients for investigations and/or the appropriate specialist where appropriate
  • Make a final diagnosis
  • Independently decide on and carry out treatment, including the prescribing of medicines
  • Make onward referrals to other services as appropriate
  • Ensure the provision of continuity of care including follow-up ;
  • Assess and evaluate, with patients, the effectiveness of the treatment and care provided and make changes as needed;

Qualifications

Essential

  • Registered Nurse with experience in urgent care.
  • Qualified Paramedic registered with the HCPC or other relevant body.

Desirable

  • A recognised qualification in Minor Injury and/or Minor Illness.
  • Independent non-medical prescriber.

Driving License

Essential

  • - Hold a full UK driving license
  • - Have access to your own vehicle

Experience

Essential

  • - Two or more years experience working as a Paramedic Practitioner, Urgent Care Practitioner and/or Registered Nurse.

Desirable

  • - Background of working in a fast-paced Primary Health Care environment.
