Job responsibilities
Primary Clinical Duties:
Accountable for the delivery of efficient, quality and evidence based clinical care in accordance to professional standards, guidelines and code of conduct (Safeguarding, Nursing and Midwifery Council, NICE, Local Authority Guidelines).
1.Management of the cohort of patients with diabetes [in line with current local and national clinical guidelines and the GMS contract
2.Work within nurse protocols as required
3.If Nurse Prescriber, it is the nurse's responsibility to adhere to national and local guidelines, protocols and best practice associated with prescribing elements of their role
4.Chronic Disease Management dependent upon training and experience
5.Maintain stock of drugs and equipment reporting to nurse management of equipment failure/maintenance requirements.
6.Contribute to clinical data collation and reporting as required.
7.Contemporaneously record all Patient Contact information on the practice medical record system
Education:
1.This role required level 1 nursing registration with the NMC and it is the post holder's responsibility to maintain all relevant elements of their professional registration.
2.The employee will be responsible for developing and sustaining own knowledge, clinical skills and professional awareness in accordance with PREP.
3.To provide ongoing advice and information to patients about their condition or provide appropriate referral if necessary.
4.To drive and encourage the participation in Diabetes education carried out in the Practice for self and junior members of the team.
5.Where appropriate, prepare results of audit and present as posters or scientific presentations at meetings and conferences.
Secondary Clinical Duties:
Accountable for the delivery of efficient, quality and evidence based clinical care in accordance to professional standards, guidelines and code of conduct (Safeguarding, Nursing and Midwifery Council, NICE, Local Authority Guidelines).
1.Routine cervical screening and contraceptive initiation and management.
2.Conduct New Patient Health Checks
3.Wound Management and wound care planning including removal of Sutures or clips
4.Long term conditions management, primarily diabetes focus but this may also include CHD; Respiratory conditions; CKD and hypertension.
5.Management of minor injuries/ailments
6.Childhood immunisations
7.Routine vaccinations and injections
8.Assist with coil and Implanon fitting and removal
Clinical Care Administration:
Accountable for ensuring efficient, appropriate and accurate patient record entries:
1.Patient Records - Accurate, efficient and appropriate Record Keeping
2.Use of templates Accurate, efficient and appropriate use of templates
3.Protocols - Accurate, efficient and appropriate use of protocols
4.QOF - Accurate, efficient and appropriate recording of clinical data as required for chronic disease management.
Other Responsibilities:
1.Accountable for maintaining a clean, safe, and efficient health care environment for the patient. Maintenance of Treatment Room Stock, decontamination after use, disposing of clinical waste and Medicines Stock Management.
2.Maintaining own knowledge and sharing clinical Best Practice: by attending appropriate training and educational events. To contribute to the development of other team members clinical skills by offering clinical support and mentorship as necessary within the Practice.
3.Planned and Unplanned Cover: To contribute in maintaining the levels of service expected of our patients and the Practice in achieving its goals by taking a fair share of covering colleagues planned and unplanned absence.
4.To ensure any CQC requirements and/or documentation for which the post holder is responsible for is completed timely and accurately. To also ensure any procedures and processes applicable to the role are followed timely and accurately.
5.Such other reasonable duties as may be delegated from time to time.
