The role:
1.
To
contribute to developing appropriate clinical networks which may be at a local,
regional or national level that improves health and social care outcomes for
patients and encourages reduced hospital admissions.
2.
To
integrate, evaluate and continuously develop physiotherapy services within the
York Integrated Care Team that improves health and social care outcomes for
patients and encourages reduced hospital admissions.
Development
of a new Physiotherapist role within York Integrated Care Team.
Responsible
for a defined area of service (physiotherapy) working without direct
supervision. Access and advice is available from senior professional colleagues
if required.
Initiate
evidence based audit and projects to evaluate and improve clinical practice
Provide
support, specialist advice and training to more junior members of staff and
students.
Provide
information and advice regarding patients to colleagues.
To
ensure a holistic approach to the overall care of patients including effective
working relationships with MDT (Multidisciplinary Team), wheelchair and
specialist equipment providers, consultants and relevant others.
3.
Provide
competent, professional and autonomous specialist physiotherapy services to
patients and colleagues within the York Integrated Care Team and encourages
reduced hospital admissions..
- Deliver
professional, competent and efficient care which meets patients health
and social needs and that promotes a reduction in the number of
inappropriate hospital admissions.
- To undertake
the specialist Physiotherapy assessment and management, of patients. This
will involve working with individuals, groups and parents / carers and
relevant others, providing specialist assessment and treatment in the most
appropriate environment.
- To undertake a
comprehensive assessment of patients, including those with diverse or complex/
multi-pathology presentations. Specialist functional assessment of
cognitive, perceptual, sensory and motor skills, which may include the use
of standardised testing. Use advanced clinical reasoning skills and
assessment techniques, to provide an accurate treatment plan and to
evaluate any intervention.
- To determine
clinical need and intervention indicated. To work as an autonomous
practitioner; holding responsibility for own caseload of patients with
complex physical, psychological and cognitive problems.
- To be
responsible for organising and planning own caseload, to meet service and
patient priorities. Readjusting plans as situations change/arise. Delegate
duties to other members of staff as appropriate.
What we look for in your application:
Essential
Proven experience
operating in a fast paced environment delivering professional and competent
physiotherapist role.
Proven experience of
succeeding in an autonomous physiotherapy role undertaking the specialist
Physiotherapy assessment and management, of patients
Proven experience of
managing own caseload of patients with complex physical, psychological and
cognitive problems.
Proven experience of
applying advanced clinical reasoning skills and assessment techniques, to
provide an accurate treatment plan for patients
Appropriate
Physiotherapist Qualification
Hold current and
appropriate registration with the Health & Care Professionals Council
(HCPC)
Suitable enhanced DBS
check
Full driving license
and access to own vehicle for business purposes
Desired
Proven participation in
clinical networks at a local, regional or national level that improves health
and social care outcomes for patients
Proven experience of
integrating and developing a physiotherapy service in a new environment /
organisation / team.
Proven experience of
integrating and developing a new to business physiotherapy role within a wider
team.
Proven experience of
delivering care which meets patients health and social needs to promote a
reduction in the number of inappropriate hospital admissions

Location: Your base will be
Tang hall lane surgery; although the nature of this role is you will be working
in the community in patient home and other settings
