Glossop Primary Care Network (PCN) are looking to recruit a full time paramedic with experience in primary care to join our team in providing a high quality service to our patients.
Main duties of the job
The role will consist of a community delivery of healthcare through acute home visits as part of the Team Up initiative.
Team Up Service Key Features
- Integrated Care Model: Brings together professionals from health, social care, and community services including advanced clinical practitioners, district nurses, social workers, home carers, and GPs to form one cohesive team.
- Home-Based Support: Focuses on delivering care directly in patients homes, reducing the need for GP visits, emergency department attendances, and hospital admissions.
- Preventative & Proactive Care: Aims to keep vulnerable individuals safe and well at home, preventing crises and supporting hospital discharge.

Clinical Duties
- Provide assessment, diagnosis, and treatment at first point of contact, either in practice or at patients homes.
- Act as an autonomous clinician, supported by GPs.
- Manage both acute and chronic conditions using drug and non-drug treatments.
- Prescribe and review medications in line with national and local guidelines.
- Support patients in adhering to treatment plans and provide preventative health advice.
Operational Duties
- Triage and prioritise patient needs, making appropriate referrals.
- Maintain accurate and contemporaneous clinical records using IT systems.
- Participate in multidisciplinary team meetings and contribute to care planning.
- Support continuity of care and reduce unnecessary GP referrals.
Professional Development
- Engage in continuous professional development and mandatory training.
- Participate in audits, clinical governance, and quality improvement initiatives.
- Uphold standards of documentation, confidentiality, and clinical safety.
