Denton, Audenshaw & Droylsden Primary Care Network (DAD PCN) are currently looking for a registered healthcare professional to support the ongoing COVID-19 vaccination programme
Hours/times negotiable ( inc term time only/school hours )
The role will be connected to the local COVID-19 vaccination hub in Denton, but will require regular travel to roving clinics/housebound patients across the Denton Audenshaw and Droylsden footprint.
Main duties of the job
DAD
PCN are looking for a qualified individual to support targeted health campaigns
such as COVID-19 vaccination, flu vaccinations and task orientated roles

Job Purpose: To work with all members of the clinical team
within the multi-disciplinary framework and maintain a high standard of patient
care under the direction of the Clinical Lead.
To work in accordance with the principles laid down by the
Nursing/Midwifery professional bodies and Trust Policies.
To support the
delivery of a high quality, safe and compassionate healthcare service, all
staff are expected to act as a role model to others in all aspects of their
work.
About us
Denton Primary Care Network is a collaboration of the primary care teams
based at 3 Denton general medical practices -
Millgate Healthcare Partnership; Guide Bridge Medical; Denton Medical
Practice, and 3 Droylsden based general medical practices Market Street
Medical, Droylsden Medical and Medlock Vale Medical Practice.
We currently have a PCN population of approximately 51,000 patients and
the PCN staff work in conjunction with the local general practices to support
both local and national priorities working closely with our local care homes,
housebound and most vulnerable patients.

To lead the vaccination process for COVID vaccinations in care homes/housebound with the support PCN staff.
To be part of the vaccination team for COVID vaccinations in PCN led clinics in the community and at the Vaccination Site.
Ensure practice is in accordance with your relevant Code of
Professional Conduct and work within limits of competence.
Maintain a high standard of patient care and clinical
interventions, applying evidence-based practice.
Maintain accurate records of all care given.
To effectively use information technology for the benefit
of patient care in accordance with the PCN's Cyber Code of Conduct.
To maintain confidentiality of information (written,
verbal, electronic), whilst being aware of the Freedom for Information Act.
Communicate effectively with patients, relatives, carers,
significant others, the multi-disciplinary team and support services.
To ensure the safe custody, supply and administration of drugs
and in accordance with local policy
To promote health education to patients, carers and
significant others.
To act as the patients advocate.
To promptly deal with equipment failure in accordance
with Trust procedure.
To demonstrate competency and teach others in the safe
and effective use of equipment in the clinical area.
To be competent in managing emergency situations
including directing other members of the team.
To be competent in clinical skills relevant to the
clinical area.
To safeguard the health and welfare of patients,
visitors and staff in all circumstances. Including management of situations of
violence or aggression.
