Job summary

This is an exciting opportunity to join our progressive team as an independent prescribing clinical pharmacist working across a primary care network (PCN)

We are looking for an experienced and highly motivated pharmacist to join our team of PCN based pharmacists and will have access to peer support and development opportunities as part of our MDT.

Main duties of the job

Applicants MUST have previous general practice experience and hold an independent prescribing qualification. The successful candidate will be required to enrol on the CPPE primary care pathway which will be fully funded or we may apply for an exemption if you are suitably qualified We are looking for a dynamic, enthusiastic experienced pharmacist to enhance our team even further. We take immense pride in our colleagues and in hand selecting our team. Each clinician must meet a vigorous internal specification and be willing to be fully adaptable with fantastic clinical acumen. We have an experienced clinical support team who can be easily accessed to provide training and support to our staff and can assist with continuing professional development needs.

About us

Denton Primary Care Network is a collaboration of the primary care teams based at 3 Denton general medical practices - Millgate Healthcare Partnership; Guide Bridge Medical; Denton Medical Practice, and 3 Droylsden based general medical practices Market Street Medical, Droylsden Medical and Medlock Vale Medical Practice. We currently have a PCN population of approximately 51,000 patients and the PCN staff work in conjunction with the local general practices to support both local and national priorities working closely with our local care homes, housebound and most vulnerable patients. We ensure all our staff have regular input and training from senior management to review any educational or development needs and to support them to meet and exceed any clinical and non clinical requirements. Essential

  • Undergraduate degree in pharmacy and registration with the General Pharmaceutical Council
  • Hold an independent prescribing qualification

Desirable

  • Qualified or enrolled on CPPE Clinical Pharmacist Training pathway (although this will be offered as part of the role)

Experience

  • Previous general practice experience
  • Knowledge and awareness of legal, ethical, professional and organisational policies/procedures relevant to the role
  • Strong clinical leadership, communication and interpersonal skills Ability to apply effective clinical judgement in the prioritisation and decision-making aspects of the role

Desirable

  • Experience in supporting common acute and long-term conditions Experience of working within a care home setting in the capacity of providing patient-centred medicines optimisation support
  • Knowledge and understanding of Primary Care Network arrangements
  • Knowledge, awareness and experience of brief intervention and referral pathways that provide opportunistic advice and support for preventable illness and/or long-term conditions receptive to lifestyle/pharmacological support
