The Denton PCN team aim is to achieve a targeted and appropriate service, delivered in accordance with evidence based practice and local policies and procedures.
Main duties of the job
The successful applicant will be supporting the delivery of care in a range of nurse led environments, utilising the Quality Outcome Framework/ Investment and Impact funding to further develop services including Complex care, Frailty management, PCN Health & Well Being clinics and support the ongoing COVID vaccination programme. The Post Holder will be expected to continually promote high standards of service within the Primary Care Network team and will work closely with the Clinical Director and Lead Nurse to develop and implement an effective Primary Care Network Strategy.

KEY RELATIONSHIPS:
1. The post holder will be required to communicate with a wide range of personal and departments including, Family and carers, GP Practice Staff, Adult Health & Social Care teams, Mental Health Services, Therapists, domiciliary care agencies, voluntary sector and Secondary Care providers.
2. They will assist with the management of both clinical and non-clinical practitioners and support Students.
3. They will be supported by the PCN Clinical Director and Lead Nurse
About us
Denton Primary Care Network is a collaboration of the primary care teams
based at 3 Denton general medical practices -
Millgate Healthcare Partnership; Guide Bridge Medical; Denton Medical
Practice, and 3 Droylsden based general medical practices Market Street
Medical, Droylsden Medical and Medlock Vale Medical Practice.
We currently have a PCN population of approximately 51,000 patients and
the PCN staff work in conjunction with the local general practices to support
both local and national priorities working closely with our local care homes,
housebound and most vulnerable patients.

PCN
nurses provide high standards of care to patients with a wide range of health
care needs. Patients living with frailty/long term health needs
should have access to well-planned, joined-up care. Care will be
delivered within the patient’s own home / other community settings. PCN nurses have
the clinical skills to complete full assessments and create detailed care
plans, including discussions surrounding Advance Care Planning. They are
required to deliver individualised and person centred care independently and report back to other members of the team, the
patient and family. The role involves
lone working; there is also GP support as well as the wider PCN/ Nurse Lead.
This is an exciting opportunity to be involved with the PCN working in
collaboration with the multidisciplinary team. As the PCN Nurse you will be
autonomous in practice and have a high level of decision-making. You will
manage your own caseload of patients registered with the six GP practices
within the Denton PCN footprint. The post holder will need to have excellent time
management and communication skills. The role also involves facilitating regular
MDT meetings within the wider neighbourhood team to support collaboration/local
strategy and the NHS Long Term plan.The PCN Nurse role
is to assess the needs of patients in order to plan and implement holistic care
within their own home (including Care Homes) They will carry out this role in
conjunction with the Neighbourhood Team and fellow members of the
multidisciplinary team.
KEY TASKS &
RESPONSIBILITIES:
1. Undertake
assessments in order to implement and evaluate programmes of care for patients
in the community.
2. Patients include
those with complex needs and long term conditions.
3. Support
palliative patients to remain in their chosen place of care
4. Refer to the
appropriate allied health care professionals as required.
5. Proactively
promote healthy lifestyle choices. Recognise situations that may be detrimental
to health referring to the appropriate agencies as required.
6. Operate within
policies and guidelines.
7. Document
accurate records of care. Maintain up to date paper and electronic notes.
8. Maintain
confidential patient information to comply with Information Governance policy.
9. Delegate to
junior team members ensuring tasks are within health and safety regulations.
10. Encourage
patients and their families to be involved in planning, delivery and evaluation
of their care through service user groups and the PALs service.
11. Work within the
multi-disciplinary team to co-ordinate and manage patient care.

mentor pre-registration students
