This vacancy has come at an exciting time following the recent merger between Heswall & Pensby Group Practice and Commonfield Road Surgery on 1st May 2024.
We are now looking to enhance our administrative workforce and have various hours/shifts available across both our sites. If you are an enthusiastic and self-motivated person with a passion for delivering excellent patient and customer care, then we want to hear from you.

Main duties of the job
The Patient Co-Ordinator will
provide general assistance to the Practice team and project a positive and
friendly image to patients and other visitors, either in person, electronically
or via the telephone.
This role is responsible for
undertaking a wide range of administrative duties which include registering
patients, booking appointments, ensuring all information is managed correctly,
filed and followed up accordingly. To act as the focal point of contact and be
responsible for the dissemination of information, messages and enquiries for
the medical team, liaising with multidisciplinary team members, other health
professionals and staff within the practice.

- Each morning; open the building, switch on
lights and computers and ensure the rooms are prepared for the day.
- Each evening; close the building and ensure all
cupboards, windows and doors are locked.
- Respond to requests for assistance from
patients, visitors and doctors.
- To support and assist clinicians as required.
- Requesting tests on behalf of clinicians via ICE
portal.
- To be responsible for the management of patient
notes in line with practice procedures.
This includes ensuring the records are complete and accurate to the best
of your knowledge.
- To be responsible for liaising with other
agencies such as the ambulance service, hospital clinics, other practices and
community services.
- Assist with ensuring staff communal areas are
kept clean and tidy in accordance with Infection Control Procedures. This may involve playing an active part in the
staff kitchen rota.
- Take responsibility for maintaining and
encouraging adequate security measures e.g. locking doors, setting security
alarm where relevant.
- Keeping up-to-date with Practice Policy and
Procedures.
- Working as part of a team whilst utilising own
initiative.
- Working flexibly when performing duties associated
to the Patient Co-Ordinator role to ensure business continuity. This may include working between multiple
Practice sites as required.
- Working flexibly to provide cover of staff
during times of absence and sickness.

new staff.
- Support Practice in achieving QOF.
- Support Practice Supervisors in the day-to-day
