Job summary
The Patient Support Service (PSS) offers patients a home-based, holistic assessment, to help address a wide range of health, social and mental health issues. The team support these patients to remain as independent as possible in their own home by accessing appropriate care, benefits, mobility aids and offering advice in many other areas that they may need help with. The team also seeks to combat loneliness and isolation by supporting the work of the volunteer Telephone Befrienders and signposting patients to suitable, as well as relevant, social opportunities.
Main duties of the job
- To work with the Senior Social Prescribers, to promote and develop the service
- To manage a caseload of patients
- To undertake patient assessments as deemed appropriate, mainly in the patient’s home
- To make referrals or signpost to statutory and voluntary services - following up as appropriate
- To maintain accurate records of each contact and share information appropriately
- To ask for help/advice if unsure how to proceed with complex patients
- To complete all mandatory training and attend other training as directed by the Senior Social Prescribers
- To work with patients to help them better understand their health and wellbeing and offer advice on ways of making improvements
- To help identify unpaid carers within practice populate and enable them to access local support services
- To work alongside the Volunteer Coordinator and the Telephone Befriending Service
- Provide coordination of and participate in relevant internal and external working groups and provide project advice, expertise and support where requested
- Support the PCN team by inputting into the overall strategy development and programming of work streams by applying knowledge and understanding of programme and project management
- Work with key personnel in the PCN to develop & support collective general practice projects including areas of federated working

The North Bucks Primary Care Network (PCN) is made up of 4 GP practices based in the North of Buckinghamshire. These are 3W Health, Waddesdon Surgery, Edlesborough Surgery and Ashcroft Surgery.
