Job summary
We are looking for an experienced Practice Nurse to work 2-3 days alongside our existing Clinical Team.
Main duties of the job
Identify, manage, and support patients at risk of developing long-term
conditions, preventing adverse effects on the patients health
Provide routine nursing care to patients as required in accordance with
clinical based evidence, NICE and the NSF
Provide vaccination and travel medicine services
Provide wound care (ulcer /Doppler etc.) to patients
Request pathology services as necessary
Process pathology results as required
Provide chronic disease clinics, delivering patient care as necessary,
referring patients to secondary / specialist care as required
Maintain accurate clinical records in conjunction with extant
legislation
Ensure read codes are used effectively
Maintain chronic disease registers
Develop, implement, and embed well woman clinics and well man clinics
Implement vaccination programmes for adults and children
Prioritise health issues and intervene appropriately
Recognise, assess, and refer patients presenting with mental health
needs
Support patients in the use of their prescribed medicines or over the
counter medicines (within own scope of practice)
Support the clinical team with all safeguarding matters, in accordance
with local and national policies
Deliver opportunistic health promotion where appropriate
Maintain baby immunisation results logs
Maintain smears results logs

Dilston is a friendly but busy GP practice based in Newcastle. We have around 9000 patients who speak over 80 different languages. We offer a range of clinics including Diabetes and Minor Surgery. We also host a number of PAMs such as Midwives, Pharmacists and Dieticians.

To

maintaining robust call and recall systems

Participate
in national screening work such as cervical cytology and deliver the national
immunisation and vaccination schedule

travel health advice and administer appropriate vaccinations

Treat
patients with minor injuries
5.
Provide
directly and assist GPs with contraceptive services
6.
Break
bad news to patients/parents/carers and provide appropriate ongoing support
to meet identified needs
7.
Undertake
Spirometry and ECGs
8.
Undertake
health promotion to help educate patients
9.
Select,
carry out and interpret results of specific tests

11.
To
maintain accurate patient records and enter onto the computer using agreed

12.
Ensure
accurate completion of all necessary documentation associated with patient
health care
13.
To
collect data and participate in regular Practice audit and research projects

To
contribute to the preparation of any Practice development plans

maintain own annual registration with the NMC and adhere to Code of

To
maintain a safe and therapeutic environment for all staff, patients, carers
