Job summary

We at RAVEN Primary Care Network are looking for a motivated and enthusiastic person to join our newly established team of Care Co-ordinators supporting our Primary Care Network of five practices in Rotherham.

The role is advertised as a full time position however the Primary Care Network is also open to candidates looking for a part time role.

Main duties of the job

The successful candidate will be supporting the practices in co-ordinating care for patients in areas such as cancer diagnosis, learning disabilities and assisting practices in supporting patients in their aligned care homes. You will join a team of three Care Co-ordinators who support and work alongside a wider range of staff working across all practices within the PCN.

The role of the Care Co-ordinator is a new and growing role within primary care and we hope for a candidate who can grow and adapt with this position.

About us

RAVEN Primary Care Network (PCN) is made up of the following five GP practices along with their branch surgeries; Stag Medical Centre, Gateway Primary Care, Brinsworth Medical Centre, Treeton Medical Centre and Thorpe Hesley Medical Centre.

The successful candidate would be expected to work across all practices and manage and support this within the existing team.
