Job summary

The Primary Care Network (PCN) Digital and Transformation Lead provides a leadership and management resource to aid service transformation, with a view to improving experience and outcomes for the local population and supporting general practice resilience.

The ideal candidate will take a proactive approach to leadership, have a passion for Primary Care, and have a proven track record in delivering service improvements.

The remit of this role is broad and requires strategic oversight as well as attention to detail in delivering high-quality day-to-day operations to support transformation.

Main duties of the job

Support the adoption of new technology and other initiatives to improve the care offer.

Enable PCN staff to work more effectively to support the sustainability of general practice services.

Lead and develop a strategic plan that includes a robust digital infrastructure.

Clarify objectives and priorities for Network service delivery in line with operational capacity, financial parameters, estates and contract requirements.

Promote the engagement of all PCN practices and act as an ambassador for the network.

Proactively build relationships with system leaders and stakeholders to help influence proactive and new models of care.

Effectively manage the PCN team to deliver high quality services across all contracts.

Use business intelligence tools to aid data-driven decisions.

About us

A31 Primary Care Network is a collaboration of three practices, providing a vehicle for transformation which supports; tackling health inequalities, improving outcomes for patients and resilience of primary care. It is already delivering a range of services to support provision of multi-disciplinary primary care. Moving forward, we intend to build on these foundations by exploring further opportunities for collaboration and broadening the scope of the digital offer.

The Digital and Transformation Lead will be instrumental in driving the change agenda for A31 PCN by developing plans and seeing them through to mobilisation and beyond. The role encompasses a variety of management skills in both a strategic and an operational context. It requires a person who can self-manage and self-motivate. The successful candidate will have previous experience of working in a senior management role and have proven skills in driving transformation initiatives.

Location

A31 Primary Care Network (PCN) is located in the North of Hampshire and consists of three GP practices covering Alton and the surrounding villages. Our total number of registered patients is approximately 32,400 patients. There are two practices in Alton (The Wilson Practice and Chawton Park Surgery) and one practice in Four Marks (Boundaries Surgery).

Lead and develop a strategic plan that includes a robust digital infrastructure.

Clarify objectives and priorities for network delivery in line with operational capacity, financial parameters, estates and contract requirements.

Promote the engagement of all PCN practices and act as an ambassador for the network.

Proactively build relationships with system leaders to help influence proactive and new models of care.

Use business intelligence tools to aid data-driven decisions.

Operations

Lead the effective adoption of national and local initiatives, including integrated working at neighbourhood and place level to improve access to services for patients.

Ensure that digital and operational PCN transformation is embedded in, and aligned with, Integrated Commissioning System (ICS) and national strategies.

Provide oversight of, and accountability for, the delivery of PCN contracts and specifications.

Line manage the PCN staff with a view to delivering high quality primary care services in line with the contract specification to support transformation and practice resilience.

Oversee training, personal development and continuing professional development of PCN staff.

Identify and record risks and issues and develop contingency plans.

Proactively drive change initiatives and project-manage mobilisation plans.

Finance and Governance

Work alongside the A31 PCN Finance Manager to:

Ensure accurate financial records are maintained.

Monitor and check the accuracy of the submissions for financial reimbursements and other funding flows

Compile and present accurate, timely and meaningful financial reports.

Ensure that the PCN's physical and financial resources are managed and maintained for maximum effectiveness

Develop and maintain high standards of corporate governance.

Digital Infrastructure

Review and improve the PCNs digital maturity

Ensure the PCN maximises its use of business intelligence tools.

Improve adoption of new technology to enhance patient access and experience and increase PCN productivity.

Use data, and improve data quality, to:

Understand demand, capacity and activity and drive improvements in:

- Operational efficiency

- Staff experience at work

Support population health management, improvements in care quality and PCN operational efficiency

Relationship Management

Build relationships and facilitate collaboration between practices to support the delivery of care to each others patients (for example; delivery of enhanced access or other hub type working arrangements).

Foster a culture of collaboration and promote learning to drive continuous improvement.

Build on established engagement with member practices Patient Participation Groups and other stakeholders to inform service development initiatives.

Develop and deliver a communication plan to promote PCN activities using a wide variety of channels.

Lead the development and embedding of PCN culture, values and reputation

Proactively engage with stakeholders, e.g. patients, staff, practices, commissioners, partner organisations, local Councils and other stakeholders, with a view to continuous service improvement and identifying future service transformation opportunities.

Represent the network in every aspect of its business operation.

HR and Workforce Planning

Oversee recruitment, induction, training, performance reviews and HR elements of PCN staff management.

Develop a coherent workforce plan to support recruitment.

Facilitate actions to promote PCN staff retention.
