Harrow
Network Partners
Harrow Network Partners Ltd (HNP) is a new
entity created to represent the 5 Primary Care Networks (PCN) established in
Harrow. Collectively, the PCNs represent the 32 General Practices operating in
Harrow and providing valuable care to over 260,000 patients.
HNP was recently successful in securing two
contracts which will run concurrently over the next 2 years.
The post
holder will take a lead role in the development of Harrow Network Partners Ltd.
They will ensure the delivery on a wide range of business, transformation and
improvement initiatives across the company providing strategic support and
leadership particularly focussing on the Harrow Training Hub and the CRM Hub
pilot.
The post
holder will develop strong relationships and engage with a wide range of
stakeholders across Primary Care in Harrow to ensure collaborative approaches
to pan Harrow development, service development and delivery.
The post
holder will be responsible for managing agreed projects with a focus upon
developing the partnerships across Primary Care and Harrow PCNs,
community services, social care, mental health, the voluntary sector and
secondary care.
Location:
Hybrid working arrangement. The post
holder may be required to work at any location within Harrow.
Hours per
week: 24-30 hours per week,
Flexible, Part time.
Length of contract: Fixed term contract
up to 31st March 2026 (extension dependent on funding)
Salary: £53,000 £60,500 (pro
rata) per annum
Main duties of the job
You must be able to work independently with
minimal guidance and use your initiative to deal with a wide variety of queries
from the Directors, using enhanced engagement skills to build strong
relationships with key contacts including community organisations,
neighbourhood leads, health development coordinators and locality leads to
ensure you are working with and as a vital part of an evolving wider view of
health and social care in a community setting. In addition, the post holder
will be expected to support the Directors in further establishing Harrow
Network Partners by-

About us
Cardio-renal-metabolic (CRM) Hub
HNP was awarded a 2-year contract to design
and pilot a clinical operating model for a cardiac renal metabolic (CRM) hub to
improve management of CKD, Diabetes and cardiovascular disease.
A successful
cardiac renal metabolic hub will aim to prevent onset, progression and
deterioration of long term conditions particularly CKD, working towards
reducing the long term demand on dialysis and transplantation, with a primary
focus on preventing progression of early disease. The initial phase of delivery to general practices will begin in Oct/Nov
2024 for the first phase of the pilot.
The post holder will be to provide effective
project management support to the Clinical Lead and guidance to the participating
practices. The effectiveness will be evaluated with the view to developing a
sustainable model that is scalable across NW London.
Harrow
Training Hubs
The Harrow Training Hub is part of the North
West London (NWL) Training Hub representing 8 local Boroughs across West
London.
Each training hub receives Health Education
England (HEE) funding targeted for use in primary care to support the
development of place-based student placements and recruitment.
The remit of the Training Hubs is to bring
together education and training resources from NHS organisations and community
providers, as well as local authorities.The post holder will be a key member of the senior
management team.

The role of Training Hubs is to:

Have a
key role in understanding, influencing, supporting, and leading educational
interventions around population health needs and health inequalities in their
area to address these.

Provide
advice on workforce planning and training needs analysis, to help find which
roles best meet the needs of the population. For example, actively supporting
workforce planning by understanding population health needs and the training
needs required to support new care pathways.

Help new
staff, appointed through the Additional Roles Reimbursement Scheme (ARRS)
scheme, embed into roles, for example supporting the roll out of the First
Contact Practitioner (FCP) programme.

Facilitate
and/or deliver opportunities for Continuing Professional Development (CPD)
programmes, based on scoping CPD requirements across PCNs.

Provide
career support at all stages for all primary care workforce.

Train
and, where appropriate, recruit more educators.

Develop
and help to keep staff through retention programmes, for example mentoring, and
preceptorships.

Support
practices and PCNs, who are looking to become learning environments, to

KEY RESPONSIBILITIES:

Pilot
KEY RESPONSIBILITIES
