We're on the look out for a Primary Care Nurse who's not just skilled, but also passionate about community health, enjoys building connections with patients, and fancies joining a team that feels more like a family.
Ideally you will also have diabetic experience and be responsible for the management of diabetes including insulin initiation,
health education, reviewing bloods, medication monitoring and dose
adjustment. Training in this area may be considered for the right candidate.
Working as part of the nursing
team, delivering care within their scope of practice to the entitled patient
population. The role will include supporting the management team in the
reviewing of clinical policy and procedure.
The post holder will deliver clinic and home-based treatment plans
including education and lifestyle advice, monitoring and management of symptoms
and medications.
The service seeks to provide access to diagnostics and early
treatment intervention. The post is integral to the multidisciplinary team,
working closely with our partners in secondary care to provide seamless
transition for patients to primary care underpinned by a solid clinical
governance structure. The post holder
will build relationships with wider external organisations to promote joined up
patient care and services.

Main duties of the job
As a practice Nurse with a focus on Chronic Disease Management, you will play a vital role in supporting patients to live well with long-term conditions. You will work autonomously and as part of the wider healthcare team to deliver evidence-based care, education, and support.
Key Responsibilities:
- Independently manage and monitor patients with a range of chronic diseases, including but not limited to:
- Diabetes
- Asthma and COPD
- Cardiovascular disease (e.g., hypertension, heart failure)
- Chronic Kidney Disease
- Conduct comprehensive health assessments, including relevant physical examinations and investigations.
- Develop, implement, and evaluate individualised care plans in collaboration with patients and the wider multidisciplinary team.
- Provide patient education and self-management advice on their chronic conditions, promoting healthy lifestyles and adherence to treatment.
- Monitor and interpret relevant clinical data (e.g., blood results, spirometry) and adjust care plans as necessary within agreed protocols.
- Administer medications as per practice protocols and patient specific directions.
- Participate in regular multidisciplinary team meetings to discuss patient care and contribute to service development.
- Keep up-to-date with current best practices and guidelines in chronic disease management.
- Undertake other general practice nursing duties as required, such as immunisations, wound care, and health promotion.
