Advanced Practitioner (Nurse, Paramedic, Physio, OT)
Hours: 37.5 per week job share and / or part-time working considered for exceptional applicants.
An exciting opportunity has arisen at Millgate Healthcare Partnership for an advanced practitioner to join our large, friendly and well established multi-disciplinary clinical team. We are seeking a motivated, enthusiastic clinician with recent clinical experience and the desire and drive to provide our patients with the highest standard of care in undertaking the wide and varied clinical, administrative and educational duties associated with this rewarding role.

Main duties of the job
- The post holder will be an experienced healthcare professional,
who acting within their professional boundaries; will provide care for the
presenting patient from initial history taking, clinical assessment,
diagnosis, treatment and evaluation of their care. They will demonstrate safe, clinical
decision-making and expert care for patients within the general
practice in alignment with the 4 pillars of advanced clinical practice.
- The post holder will work collaboratively with the
multi-disciplinary general practice team to meet the needs of patients,
supporting the delivery of policy and procedures.
- The post holder will build effective working relationships with
other health care professionals.
- The post holder will work within the team of GPs, Advanced
Clinical Practitioners, nurses, paramedics, pharmacists, physician
associates and health care assistants to deliver excellent primary health
care services within the practice.
About us
Millgate Healthcare Partnership is a relatively young organisation formed following the merger of Churchgate and Windmill Medical Practices. We currently operate 5 days per week between the hours of 7am and 8pm (on selected days). As a team we are committed to remaining a key central component in the local community serving and interacting with those around us.
We are a high-achieving QOF practice, closely involved in local Primary Care Networks, GP-led clinical commissioning group and are the lead practice locally for GP and AHP education and training. As an enhanced training practice we have committed to maintaining a strong focus and commitment on developing our staff and training junior doctors and AHPs.
Our team comprises GPs, Practice Managers, ANPs, Pharmacist, Practice Nurses, Assistant Practitioners, Healthcare Assistants and administrative staff. We were recently rated as good following our first CQC inspection as a merged practice.

- Evidence of advanced critical thinking skills, clinical decision making and assessment and management of clinical risk
- Autonomous practice qualification
- Clinical assessment qualification at Min of Academic Level 7
- Non-medical prescribing qualification
- Masters level qualification (exceptional candidates due to qualify will be considered for interview)
Desirable
- Management qualification
- Mentorship or teaching qualification

- 1 year recent relevant experience at AFC band 6 or above
- 5 years post qualificatory clinical experience.
- Familiar with the interpretation of diagnostics and principles of clinical decision making.

- Undertaken safeguarding qualification

- Experience of teaching at informal and formal level
