Clinical Care Coordinator

Job summary

The post holder will provide Clinical Care-Coordination for the population of Lancaster Medical Practice and Lancaster Integrated Care Community in line with the Better Care Together Strategy.

The post-holder will be pivotal to the ICC and will work alongside the Practice and other providers.

This role requires a flexible approach, willingness to try things out, and an ability to accept a changing remit.

The Integrated Care Community is a multidisciplinary team with a geographical footprint based within primary care communities. The aim of this team is to proactively manage clients, to prevent hospital use and allow early hospital discharge. The team model places the client and their experience firmly at the centre of the model and encourages self-support. The ICC wraps around the client and offers a simple interface where the client is able to access the right support, in the right place, at the right time.

The role is for 24 hours a week on a 12 month fixed term contract. The closing date for applications is 30 April.

Main duties of the job

The post holder will be based at Lancaster Medical Practice but the work involves various locations across the locality and will work both as part of the Lancaster PCN Team and a wider multi-disciplinary group, playing a lead role in coordinating support and support to all clients within the ICC. They manage a short term caseload of identified clients, to ensure that appropriate support is delivered and that unplanned, changing needs are addressed. The post holder is expected to act as an advocate for clients.

Please note, we are unable to support candidates requiring a skilled worker visa for this post as the Practice is only able to support clinical posts at this time.

  • Carry out initial screening of individual clients to develop and agree a Support Plan containing actions covering the short and medium-terms. Where necessary identifying urgent needs taking relevant action or liaison
  • To implement the Support Plan by setting up services using the appropriate procedures and systems and coordinating health and social care support that is being delivered, ensuring it meets the needs of the individual and is client centred.
  • To provide interventions such as advice, guidance and direct support to clients on their caseload
  • To refer and liaise with a range of voluntary sector support agencies, ensuring the client has access to a wide range of support meeting fully their holistic needs
  • Contribute to overarching interdisciplinary support plan and other case management tools by participating in meetings; coordinating information and support requirements with other support providers, and reducing overlap/repetition with other practitioners
  • Acting as a single point of contact for clients on their caseload, their families and other professionals involved in the support of the individual
  • Effectively communicate with external and internal colleagues involved in the support of the client, particularly with the GP and practice staff
  • Carry out regular reviews of the Support Plan, adapting and evaluating interventions that support the client, adjusting own approach accordingly
  • To input relevant information into client records, ensuring complete and accurate case records are maintained
  • To take part in the evaluation of the role
