Job summary

We have an exciting opportunity for a Nursing Associate to join Lancaster Medical Practice. The post holder will help to provide and maintain a high standard of nursing care for patients, as well as providing nursing assistance to Practice Nurses, Doctors and other members of the primary healthcare team. The post holder will understand and act accordingly in line with the NMC professional standards.

The successful candidate will also work alongside our Frailty Matrons as part of our visiting service and may be required to undertake home visits as part of their role.

Main duties of the job

Understanding and assessing clinical risk and reporting any ill health of patients and make requests for GPs to see the patient where necessary.

Monitoring and support in the management of patients with long term conditions/chronic conditions, using holistic patient centred approach.

Perform and record clinical observations such as blood pressure, temperature, respirations and pulse, for example as set out in the NMC standards.

Take vital signs and observations and identify signs of improvement, report any deteriorations or concerns, e.g. pulse, blood pressure, respirations, oxygen saturations, peak flow, blood glucose levels, ECG, assessing nutrition and hydration.

Basic wound care management.

Administer medications as prescribed and in accordance with NMC standards, e.g. via nebuliser or injections using sub-cutaneous or intramuscular route under Patient Specific Directions.

Following agreed clinical protocols with referral to Senior Nurses or GPs as appropriate assist in the assessment, planning, development, implementation and evaluation of programmes to promote health and well-being and prevent adverse effects.

Deliver opportunistic health promotion using opportunities such as new patient medicals and NHS Health Checks.
