Job summary

· To acquire the required academic standard, underpinning knowledge and technical skills to obtain a BTEC Level 3 Diploma in Pharmacy Service Skills.

· Develop a portfolio of evidence as required

· Work closely with team members to achieve workplace assessments

· To undertake, with supervision, all types of medicines management and community pharmacy work appropriate to the level of knowledge and experience, in order to deliver services to patients

Main duties of the job

Repeat Prescribing/Medicines Optimisation/Community Pharmacy – these skills will be developed during the two year training period

Work with the Lead pharmacists to improve efficiency and effectiveness of repeat prescribing

Identify patients suitable for medication changes in line with the Medicines Optimisation Strategy and subsequently make those changes with the agreement of the Pharmacists.

Support medication reviews. Support the Practice Pharmacist/Doctor/Nurse carrying out medication reviews or clinical audits.

Liaise with patients, practice staff, GPs, care homes and Community Pharmacists in all aspects of medication reviews

Responsible for informing patients of any changes to their medication.

Use Docman/EMIS workflow to update patient medications from hospital discharge letters, clinic letters etc

Accurate dispensing of prescription medications and appliances to the customers of Rosebank Pharmacy, following the required protocols and procedures as directed by a licensed Pharmacist.

Preparing prescriptions by reviewing prescription information, measuring medications, mixing or compounding medications and attaching the appropriate labels.

To collect all prescription charges and ensure the FP10 patient declaration is completed and signed.

Operate efficient stock ordering system appropriate to the needs of the Pharmacy.
