Job summary

You will be joining an established team of nurses who work alongside the clinicians in our modern purpose built practice. Serving 15600 patients we are a training practice with GPs who value diversity, innovation and commitment. Our friendly team consists of 4 partners 6 salaried GPs, 3 pharmacists, 2 nurse practitioners, 3 prescription clerks, 2 pharmacy technicians and a supportive management and administrative team. We believe in a close working partnership where ideas are shared, you are listened to and the health and wellbeing of staff is paramount.

We offer training relevant to career progression and our lead nurse is a mentor for newly qualified nurses and also for those from secondary care who wish to move into primary care. We are actively seeking a nurse with some experience of managing diabetes, as one of your days will be spent seeing this cohort. The other 2 days will be covering all aspects of practice nursing-wound care, cytology, immunisations etc. so this role should suit a nurse who likes variety in their role.

Main duties of the job

The role will encompass all aspects of general nursing plus diabetes care.

  • Diabetes
  • Dopplers and wound care including complex dressings
  • Cervical smears
  • Immunisations (child and adult)

They have been a great success. We are all hands-on in team spirit. No-one is any more important than anyone else. We work hard but also enjoy social events, lunchtime walks, support from our HR health and well being champion and the partners ensure they get to know everyone- our photo board in reception helps!

The following are the core responsibilities of the practice nurse. There may be on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels:

a. Provide routine nursing care to patients as required in accordance with clinical based evidence

b. Provide wound care (ulcer /Doppler etc.) to patients

c. Provide immunisations (child and adult)

d. Request pathology services as necessary and Process results as required

e. Support an established clinical team with Diabetes care for those on our diabetes register.

f. Maintain accurate clinical records and ensure read codes and templates are used effectively

g. Support the team in dealing with clinical emergencies

h. Recognise, assess and refer patients presenting with mental health needs

i. Implement vaccination programmes for adults and children

j. Liaise with external bodies to ensure patients are supported appropriately (vulnerable patients etc.)

k. Support the clinical team with all safeguarding matters, in accordance with local and national policies

l. Understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately

In addition to the primary responsibilities, the practice nurse may be requested to:

a. Monitor and ensure the safe storage, rotation and disposal of medicaments

b. Support junior members of the nursing team, providing guidance when necessary

c. Participate in local initiatives to enhance service delivery and patient care

d. Support and participate in shared learning within the practice

e. Continually review clinical practices, responding to national policies and initiatives where appropriate

f. Participate in the review of significant and near-miss events applying a structured approach

g. Chaperone patients where necessary
