Due to retirement an exciting opportunity has arisen at Baddeley Green Surgery for full time or part time considered Medical Administrator/Care Navigator. We are looking for an experienced Primary Care Administrator/Care Navigator who is pro-active, enthusiastic self-motivated candidate with experience in dealing with the general administration duties, general public face to face and via telephone, in either a previous reception or customer service role.
Main duties of the job
To be responsible for undertaking a wide range of administration and care navigation duties providing support to the multidisciplinary team. Duties can include but are not limited to, documentation workflow, greeting and directing patients, effective use of the appointment system, booking appointments, referring patients, scanning and processing of information and assisting patients as required. To act as the central point of contact for patients, the distribution of information, messages and enquiries for the clinical team, liaising with multidisciplinary team members and external agencies such as secondary care and community service providers.
To provide effective high quality and responsive care appropriate and timely within a safe environment to meet the needs of our registered patients without restriction or discrimination, adhering to information governance regulations at all times.
To ensure all staff both clinical and non-clinical are aware and up to date with all relevant training for their roles, to ensure safe delivery of services to patients.

Job responsibilities
Maintaining and monitoring general administration tasks, Process personal, telephone and e-requests for appointments
Answer incoming phone calls, transferring calls or dealing with the callers request appropriately
Signpost patients to the correct service
Initiating contact with and responding to, requests from patients, team members and external agencies
Support in the management of repeat prescriptions, ensuring they are processed accurately and efficiently
Action incoming emails when necessary
Photocopy documentation as required
Data entry of new and temporary registrations and relevant patient information as required
Input data into the patients healthcare records as necessary. Carry out system searches as requested
Maintain a clean, tidy, effective working area at all times
Monitor and maintain the reception area and notice boards
Support Admin, Secretarial, clinical staff with general tasks as requested. Support administrative staff, providing cover during staff absences
Scanning of patient related documentation and attaching scanned documents to patients healthcare records
Referring patients to secondary care
Complete opening and closing procedures in accordance with the duty rota
Ordering and monitoring of stationery supplies
