Job summary
We are looking for a Clinical Pharmacist to join our well established Primary Care Pharmacy Team. The Team consists of two Senior Clinical Pharmacists, a Care Home Pharmacist, two Clinical Pharmacists and three Pharmacy Technicians.
Full time hours are available but we would also consider part time hours although Thursdays and Fridays are essential days to ensure good coverage across the week.
Main duties of the job
There are two main elements to this role.
One is supporting the practices with safe prescribing, providing expertise in clinical medicines advice, acute prescription requests, medicines reconciliation and risk stratification of identified cohorts.
The second is direct patient contact (usually by telephone) for structured meds reviews, clinical guidance, DOAC reviews and other queries.
