Job summary

Wyre Integrated Network PCN is looking for a Primary Care Network Digital and Transformation Lead to Join their team. This is an exciting role that will lead transformation work across the PCN and use Digital where appropriate to enable the change and maximise the use of the tools available. The Digital and Transformation lead will work with clinical and operational teams within the Primary Care Network (PCN) to improve patient access, support the integration of technological solutions to help practice teams manage workload, improve staff and patient satisfaction, and the efficiency and sustainability of the PCN and practice services.

Main duties of the job

The successful candidate will help the PCN adopt new technology to improve access to care and enable PCN staff to work more effectively to support the sustainability of general practice services.

The objective of the Digital and Transformation Lead is to support with the development and delivery of a strategic plan to build a robust digital infrastructure for Wyre Integrated Network PCN, which will involve;

  • Underpinning a quality improvement methodology
  • Integrated working
  • Reduction of variation between practices
  • Easy-to-understand data packs to support decision-making that includes but is not limited to demand, capacity, utilisation data, staff and patient experience
  • Builds staff skills and capabilities
  • Enhances patient access whilst protecting our workforce
  • Takes into consideration the digital maturity of the PCN
  • Takes into consideration the population's health needs and health inequalities within our community
  • Improves digital accessibility for our patients

WIN PCN covers 29,000 patients over four practices in the Thornton-Poulton area and uses a network of skilled clinicians to forge relationships with varying healthcare providers and promote collaborative working through the 4 partner practices, providing personalised care for patients through every stage of life.

Leadership and Management

  • To develop and maintain an in-depth understanding of the PCN digital infrastructure and business intelligence tools
  • To procure and manage digital tools for the PCN which is aligned to a framework of fundamental principles
  • To proactively project manage all digital projects and also evaluate the effectiveness of the intervention
  • To ensure all initiatives are supported by a clinical champion and practice management
  • To lead the development and implementation of digital transformation bids
  • To ensure all staff receive training on the digital tools within the PCN
  • To ensure the induction of all new members of staff receive training and resources on tools available
  • To ensure all initiatives are fully scoped and underpinned by a quality improvement methodology
  • To remain up to date with the national and local PCN guidance.

Governance

  • To create and maintain accurate documentation in relation to finances, standard operating procedures, contracts, contacts and decision-making, which is accessible centrally.

Communication, relationship building and presentation skills

  • To work in partnership with the PCN leadership and management team
  • To identify and develop relationships with practice staff, private providers, the local primary care team, neighbouring PCNs and other key partners to enable good working relationships and promote and share best practice
  • To deliver technical information in a non-technical and engaging way to a range of stakeholders

  • Experience in Digital / IM&T project management or transformation experience in a Health or Social Care Environment
  • Experience in project delivery through the entire project lifecycle from initiation to delivery
  • Experienced in delivering multiple programmes of IT projects through the entire project lifecycle.
  • Good understanding of the Digital agenda within the NHS and Social Care or Care Sector
  • Experience in delivering IT projects across multiples sites and departments
