An exciting
opportunity has arisen for an experienced Nurse Prescriber/Enhanced Practitioner with an interest in mental
health and substance misuse to work in a multi-disciplinary and multi-agency
primary care setting in North Cumbria. Applications are welcome from
experienced Nurse Prescribers.
As an
experienced and competent practitioner there will be the opportunity to work
within the primary care setting utilising a mental health service model. This
offers an essential service for those patients presenting at GP practices with
a variety of mental health problems and difficulties with substance misuse.
Main duties of the job
GP surgeries are often the first point of contact for people seeking mental health and support with substance misuse. The practitioner would assess, treat and signpost those experiencing mild, moderate, and acute mental health problems.
The role enables people to access responsive interventions close to home in their community.
As part of a multi-professional team, the post holder will practice autonomously, whilst being accountable and self-directed in line with the relevant code of professional conduct.
To provide expert clinical advice, leadership and support ensuring the needs of the service are met by exercising and demonstrating high levels of clinical judgement, critical analysis and advanced decision making skills.
Monitor and improve standards through supervision, evidence based practice, clinical audit, research and education.
Link in with other local services to improve access to appropriate healthcare. Have a good understanding of what services and resources are available to effectively signpost to a range of community-based options.
Complete specialist holistic assessment with patients supporting shared decision making including self-management. Evidence based brief biopsychosocial interventions will be provided where appropriate.
Work collaboratively with patients, carers, and their families networking with the wider multi agency team such as GPs, Practice Nurses, Pharmacists, Social Prescribers, Recovery Steps, Voluntary and Third Sector Organisations.

Experience
Essential
- Awareness and understanding of professional accountability, NMC or equivalent and local/national initiatives
- Long term (chronic) disease management experience
- Experience of implementing evidence based practice
- Team leadership abilities and experience
- Experience in caseload management
- Health promotion experience
Desirable
- Understanding of difficult to manage/challenging behaviours
- Experience of working in primary care
Qualifications
Essential
- Registered nurse with current NMC registration
- Minimum of 3 years experience at a Senior Nurse demonstrating Leadership and Management skills. Nurse Prescriber Qualification.
- Risk Assessment and Risk Management / Safety Planning skills for individuals with mild, moderate and complex mental health needs
- Demonstrate excellent communication and interpersonal skills and an ability to relate positively with a wide range of professional and non-professional staff and to manage conflict and difficult situations
