Clinical:
To work autonomously and be accountable for own professional actions.
Undertake Clinical Nursing Practice at an advanced level using expert knowledge and clinical skills
Deliver holistic care to people accessing primary health care services.
Assess, diagnose and treat patients attending primary health care services with a range of acute, non-acute and long-term medical conditions.
Assess, diagnose and treat patients in surgery or own home who require acute medical attention and refer to appropriate agency.
Directly refer patients to secondary care in acute medical need.
Refer patients to other medical specialities for assessment when appropriate.
Refer patients as appropriate to other members of the multidisciplinary team, secondary care and to other statutory and voluntary organisations including referral for investigations
Interpret a range of diagnostic tests and routine clinical procedures.

Make critical judgements to satisfy the expectations and demands of the job
Advise and support other junior colleagues.
Work within professional guidelines and codes of conduct.
Work within all relevant practice policies and procedural guidelines e.g. infection control, chaperoning, risk management.
Be an Independent Non-Medical Prescriber.
CARE AND PROGRAMME MANAGEMENT
Develop and manage care and provides highly specialised advice concerning care normally beyond the scope of normal Nursing practice. Consult, assess, establishes diagnoses, implements and evaluates management/treatment plans and makes referrals as appropriate. Makes fast track referrals.
Communicate highly sensitive condition related information to patients, relatives and carers.
Initiate the process of diagnosis with patients suspected to have a chronic disease
Contribute to practice targets both local and national within the disease management
Audit outcomes of care against standards and initiate changes as necessary.
Undertake annual reviews of long-term conditions
Refer to other members of the extended primary health care team as appropriate
To actively lead on and implement safeguarding protocols for children and vulnerable adults and to know who to refer to and how to escalate concerns. Be the point of contact in the practice for safeguarding matters and communicate with the social services for the safety of the vulnerable in our community
To lead on Childrens Immunisations as directed by national set targets
STRATEGIC DEVELOPMENT OF PRIMARY CARE SERVICES
Contribute to the strategic development of primary care and public health services to meet patient and population needs within the practice. Work/liaise across multi-professional and agency boundaries.
Work at the forefront of practice innovation. Determines how to deal with ambiguous or unique problems to develop and advance primary care services for the benefit of patients, carers and communities.
Recognition and involvement in Morris House Group Practice as a teaching practice and help drive the Nursing, paramedics, PAs, medical Students and GP trainees training and development.
Develop links within the Primary Care Network and other local and national bodies relevant to primary care Nursing initiatives.
HEALTH IMPROVEMENT
Initiate and carry out programmes of health screening and ensures other team members are aware of health agendas and screening programmes.
Introduce innovation in practice to meet ongoing health need demands of the practice population.
Work to introduce new developments that will improve health outcomes for patients

Share innovation and new learning with others.
Support practitioners in profiling the practice population in order to initiate contribute and promote improved health and prevent disease in individuals and groups.
Represent individuals and families interests when they are not able to do so themselves.
Make referrals to statutory and voluntary agencies as appropriate.
COMMUNICATION
Provide and receive highly complex, sensitive information and frequently deals with situations which are complex.
Use developed communication, negotiation and management skills
Regularly communicate service-related information to Partners, Practice Manager, Salaried GPs, Nursing, AHPs, Administrative staff, patients, relatives and carers.
Make operational judgements, manages conflicting views and reconciles inter and intra professional differences of opinion.
Build and communicate therapeutic working relationships with a wide array of statutory and voluntary organisations for the benefit of patient care and facilitates good working relationships.
Develop and deliver presentations to groups
MANAGEMENT & LEADERSHIP
Develop a special interest (dependent on experience) within the practice
Encourage and motivate others to be forward thinking in ideas that address the health needs of the practice that will be innovative whilst managing and developing new ways of working,
Develop and lead on evidence-based practice, standards and promote best practice.
Promote effective communication and relationships within the team.
Actively participate in clinical and practice meetings.
Develop own knowledge and practice to meet objectives/changes in service, through attendance on study days, self-directed learning and reflection on practice.
Integrate theory into practice and establish formal links with educational institutions.
Assist in the mentorship, assessment, appraisals.
Act as mentor for any students and other staff within the practice.
Operate high standards of probity.
Complete mandatory training in a timely fashion
Assist the practice achieve CQC targets
INFLUENCE AND DEVELOP POLICY
Implement and adhere to practice/local/national, clinical commissioning group protocols and pathways, propose developments and improvements to service delivery.

MANAGE AND USE INFORMATION
Make effective use of the clinical system by ensuring the consistent and accurate entering of data and use of Read Codes.
Facilitate the use of up to date information technology in order to collate accurate and timely information as and when required by the practice.
Ensure a satisfactory call and recall system of patients.
Assist in the development of practice protocols for the surgery and clinical protocols for the clinical team.
Identify, monitor and review deficiencies in procedures and implements remedial action.
