We have strong relationships with the PCN, including a community rehabilitation team, district nurses, an integrated frailty team, pharmacy technicians and have access to the local community hospital but are not required to staff it.
Monthly virtual ward meetings and close contact with hospice nurses for palliative care patients, means we have multidisciplinary input as needed. We have a daily catch up over coffee and as a small practice it is easy to discuss patients with colleagues.

Main duties of the job
We are offering four clinical sessions over 2 days. We feel that work-life balance is paramount & as part of healthy work-life balance, GPs have a mixture of 15 minute face-to-face appointments, 10 minute phone-calls & a maximum of 22 appointments per day, less than the current BMA recommendations. There will be no out-of-hours commitment & extended hours are run by the PCN. There will be involvement in the duty doctor rota, in hours.
The duty day mon-fri is run by our two fantastic ANPs, freeing the routine slots up for GPs. So, duty GP mon-thurs really involves actioning anything urgent, but otherwise running a routine surgery. The friday duty is on an alternating rota with the salaried GPs.
We have been building a fantastic team of clinicians & a highly skilled practice team in the last year and would welcome salaried GP to our team!

We are a small friendly practice of 5100 patients, with the added benefit of being a dispensing practice. GP administration burden has been reduced through an in-house workflow administrator who processes all paperwork for scanning and coding. In addition, we have additional remote GP telephone appointments, as well as an in-house clinical pharmacist and first contact physio provided through the PCN.

- Consultations, administration, complying with the practices contract type (for example, GMS/PMS/APMS),
- Education and training delivery, and private contracts.
Key responsibilities clinical
Caring for patients
- Practice evidence-based medicine, in line with national and locally agreed guidelines
- Conduct patient consultations during the practices clinical appointment hours
- Triage patients presenting to the practice in person, via eConsult and by telephone (if covering the on call in ANP absence or part of GP duty rota)
- Conduct telephone consultations and home visits as appropriate (very minimal visits at surgery 0-1 on average)
- Check and sign repeat prescription requests.
- Check, manage and process patient test results for your patient list/requests
- Prescribe in line with local and national guidelines.
- Refer patients to secondary/urgent care and additional interventions as appropriate.
- Manage long-term conditions and patients with complex problems.
- Promote healthy living strategies and give health and wellbeing advice, generally encouraging patients to follow a healthy lifestyle.
- Carry out patient testing and screening as required.
- Make autonomous clinical decisions relating to patients health needs.

- Use communication that is appropriate to the situation, including communicating sensitively with patients and carers when delivering bad news.

- Provide services to the practices patient population, to the highest possible standard, in line with competencies and professional code of conduct.
- Monitor and maintain the standard of care provided, taking appropriate action if standards are not met, through self- and peer-review, benchmarking, and evaluation.
- Lead and undertake clinical governance and clinical audit activities to ensure the continual improvement of service delivery (if appropriate).
