Northern Moor Medical Practice is recruiting for a part-time (2 days per week) Practice Nurse to join our well-established and friendly team, working alongside our current practice nurse, HCA and phlebotomy service.

Main duties of the job
The Practice Nurse will be responsible for providing a safe and effective clinical nursing service to
our patient population, including treatment, chronic disease management and healthcare
advice.
Managing a diverse clinical
caseload using autonomous clinical skills and broad in-depth theoretical
knowledge.
Play an integral part in
contributing to audits, research and evaluation of local standards, policies and
guidelines.
All duties to be carried out
in accordance with the NMC Code of Professional Conduct and with regard to
practice standards and protocols.
About us
Northern Moor Medical Practice is located in South Manchester on a leafy, residential road opposite Wythenshawe Park, and within easy access to transport links.
We are a training practice for GP Registrars and medical students, and we form part of the Northenden and Brooklands PCN. The practice is led by 3 Partners, with a wider team including 3 salaried GPs, 1 practice nurse, an HCA, 4 GP registrars, supported by an administrative/reception team of 10 staff members. In addition, our ARRS team includes a pharmacy technician, physician associate, first contact physio and care co-ordinator.
We have a list size of nearly 6,000, and which is growing year-on-year.

Main areas of responsibility:
The delivery of practice nursing care to our patient
population, with a focus on patients presenting with long term conditions as
well as providing preventative health care and promoting health and wellbeing
Deliver specialist primary nurse led services, including
chronic disease management (all areas of QOF) and enhanced service targets
Perform cervical screening and promote breast awareness
Administer adult and paediatric vaccinations, ensuring safe
storage, rotation and disposal of vaccines and drugs
Provide advice on lifestyle issues, assist and support
patients in changing their behaviours and refer appropriately
Recognise signs of child abuse, family violence, vulnerable
adults, substance abuse, addictive behaviours, suicidal tendencies and
demonstrate awareness of local referral procedures
Apply infection control measures when handling laboratory
specimens, disposing of clinical waste, dealing with blood and bodily fluid
spillages and follow guidelines in respect of needlestick injuries
Contribute to the development of policies, protocols and
guidelines within the practice
Undertake annual training requirements, and promote the
continual professional development and skills of both yourself and other nurses
within the practice
Attend and contribute to both internal and external
meetings, as required
Work within the latest NMC Code of Conduct, and maintain
professional registration
Ensure all data protection requirements are met when
gathering, recording and storing patient data
Working collaboratively within the wider practice team to
support the delivery of the very best patient-centred care, using effective
communication, support and cooperation
