To provide high quality, holistic, individualised diabetes care to patients in a Primary Care setting.
Job Responsibilities: (as qualified to do so)
- Competent in performing comprehensive diabetes reviews with referral on as necessary. It is desirable thatyou wouldhave completed training in insulin and GLP1 management
- Promote a greater understanding of diabetes and empower patients and their care givers through education, support and counselling
- Use evidence based practice and patient centred goals to assess, plan, implement and evaluate interventions
- Confidence in altering diabetes treatment regimes
- Work collaboratively and with shared decision making
- Confidence in initiating and reviewing continuous glucose monitors is desirable
- Experience in group diabetes consultations is desirable
- An ability to request, interpret and action pathology tests related to diabetes and its complications
- Follow agreed clinical protocols with referral to senior nurses or GPs as appropriate
- To assist in the management of clinical indicators (QOF) and update patient records at every opportunity
Main duties of the job
- Provide nursing treatments and procedures to patients in participation with GPs or independently to agreed protocols
- Provide general and specific health screening to the patients (within agreed protocols) with referral to GP as necessary
- Advise patients on general health care and promote disease prevention opportunistically
Administration
- Participation in administrative systems in the Practice
- At all times there will be a need to maintain accurate records
- A duty to advise senior nurses of potential problems or errors within the range of assigned tasks
- Attend and participate in any Practice meetings when required.
- Any other delegated duties appropriate to the post
