Job summary

We are seeking a Practice Nurse with Chronic Disease management experience to join our nursing team and we are looking for candidates with a general practice nursing background. In order to continue with our excellent long term condition management we are looking for a candidate with an interest in Diabetes or Respiratory care, but for the right candidate we are willing to support training in this area.

The position is full time, but we would consider splitting the job between 2 candidates if your preference is for part time hours. The hours to work are from 8am to 6.30 pm, but we would could split this into mornings only or afternoons only, or 2 days, or 3 days as examples of options to consider.

We envisage candidates having to give 2 months notice, so we have indicated a potential start date of the 1st March 2024 but we are very happy to consider candidates who are available to start earlier.

Main duties of the job

Job Summary:

To assist medical personnel in the care of the patient to include treatment, preventative care, screening and patient education.

Job Responsibilities: (as qualified to do so)

  • Assist in and perform routine tasks related to patient care as directed by senior nursing staff and GPs
  • Cervical cytology
  • Contraception
  • Wound care/ulcer care/Doppler
  • Hypertension Management
  • Routine immunisations/Childhood immunisations
  • Chaperoning and assisting patients where appropriate who are being examined by another clinician
  • Venepuncture
  • Removal of Sutures
  • ECGs
  • Requesting pathology tests, for example urine culture, swabs
  • Provide general and specific health screenings to patients (within agreed protocols) with referral to GPs as necessary
  • Following agreed clinical protocols with referral to senior nurses or GPs as appropriate
  • To assist in the management of clinical indicators (QOF) and update patient records at every opportunity.
  • Participate in the Management of Chronic Disease Clinics (as qualified to do so)
    • Asthma clinic
    • COPD Clinic
    • Diabetes clinic

  • Post graduate diploma or degree (Primary Care)
  • Diabetes/Asthma/COPD qualification
