Job summary

The Project surgery is looking for an enthusiastic (band 4 agenda for change) Health Care Assistant to join our small, supportive and award winning team.

We are an innovative practice and our lead GP is the current GP of the Year. Our ground breaking drive through clinic for delivering vaccinations during covid, has been nationally recognised.

We are hoping you will work with us full time.

We have a young population and a list size of 5000 patients.

We are the lead practice in our primary care network and you will have lots of leadership development opportunities.

We are situated 2 minutes away from Plaistow tube station to make your travel easier.

The vibrance of Stratford and the Olympic Park is 20 minutes away

Main duties of the job

  • Long Term Condition Reviews (ie diabetes, hypertension)
  • NHS Health Checks
  • Primary Prevention Reviews
  • New Patient Registrations
  • ECG's
  • Assisting with 6 week baby checks
  • Flu Clinics

About us

You will be supported by a practice nurse and 3 other GPs as well as our friendly admin team.

QOF targets and local extended primary care service achievement is high.

Job responsibilities

Job description

The HCA will work as part of the nursing team

Minimum qualifications will be NVQ level 3 or the Care certificate

Duties

Clinical

The HCAs role will be to data gather and inform the GP if results are abormal eg if blood pressure is high (they will work to protocols)

They are not expected to initiate treatment or make diagnoses

The HCA will undertake QOF and local enhanced service reviews, they will also do public health reviews

1.diabetes QOF reviews

2.hypertension reviews

3.COPD reviews including spirometry

4.asthma reviews

5.CHD and stroke reviews

6.mental health reviews

7.LD reviews

8.mental health reviews

9.vascular risk assessments (health heart checks)

10.new patient registrations

11.latent TB IGRA blood forms

12.6 week baby checks

13.ECGs

14.prediabetes/primary prevention CVD

Non clinical

1.fridge temperature daily monitoring and 3 monthly audits checking data loggers

2.drug stock checks monthly

3.vaccine ordering

4.clinical supplies ordering

5.room stock checks

6.calibrating VRA machines and glucose machines#

7. crashbag checks

8.FP34Ds returns

9. stock check clinical cupboards
