Job summary
Calne and Yatton Keynell PCN are looking for an experienced clinical pharmacist, ideally IP qualified.
We recognise the importance of Clinical Pharmacists in Primary Care and the key role that they have is supporting the delivery of the Network Contract DES and Quality Outcomes Framework (QOF). Delivering a range of health interventions at Practice level, including developing and managing medicine optimisation services within Primary Care, systems for safer prescribing, structured medication reviews (for example of patients with polypharmacy), reviews of patients with long-term conditions that fall within their competency, withdrawing medicines no longer needed through NHS led programmes.
You will work to improve communication about medication-related issues between the practice and other care providers and to lead on implementation of safe and efficient repeat prescribing systems. This is a new and exciting role for you to develop and strive to support the needs of the PCN going forward.
Main duties of the job
The post holder will provide primary support to general practice staff with regards to prescription and medication queries. They will help support the repeat prescription system, deal with acute prescription requests, and medicines reconciliation on transfer of care and systems for safer prescribing, providing expertise in clinical medicines advice while addressing both public and social care needs of patient in the GP practice(s).
The post holder will provide clinical leadership on medicines optimisation and quality improvement, manage some aspects of the quality and outcomes framework and enhanced services and contribute to the delivery of the local medicines optimisation strategy.
The post holder will ensure that the practice improves patient outcomes. The role is pivotal to improving the quality of care and operational efficiencies so requires motivation and passion to deliver excellent service within general practice.
The post holder will be supported to develop their role to become a nonmedical prescriber (if not already a prescriber).
