Are you passionate about supporting people to improve their wellbeing and make positive changes in their lives?

Were looking for a Social Prescriber to join our friendly and forward-thinking team at Avon Valley Practice, working across our Upavon and Durrington surgeries.

This is a rewarding role supporting patients mental wellbeing and helping individuals connect with the right community services, activities, and practical support. The postholder will take a lead in supporting carers and military veterans, as well as following up with patients whofrequentlyattend thePractice, ensuring they get the right help at the right time.

Main duties of the job

Whatyoulldo

  • Work closely with our GPs,nursesand wellbeing team to support patients social,emotionaland practical needs.
  • Focus on improving mental health and resilience, reducing isolation, and empowering people to take control of their wellbeing.
  • Lead on Carers and Armed Forces Veteran initiatives within the practice.
  • Follow up and provide tailored support for high-frequency service users, helping to reduce avoidable GP appointments.

Working closely with clinical and non-clinical teams, the postholder maintains accurate records, builds strong community links, undertakes required training and supervision, and contributes to PCN wellbeing initiatives to improve patient experience and access to support.
