Job summary
Exciting opportunity has arisen in Salford South East PCN Ltd. We have a vacancy for a second care coordinator to fill to join our innovative & forward thinking PCN at Clarendon Surgery
The successful candidate will play a key role in proactively identifying and working with people, including the frail/elderly and those with long-term conditions, to provide coordination and navigation of care & support across health & care services.
You will work closely with the PCN, GPs & practice teams, making sure that appropriate support is made available to people; supporting them to understand & manage their condition & ensuring their changing needs are addressed. They will enable people to access the services & support they require to meet their health and wellbeing needs, helping to improve peoples quality of life. You will also be supporting the early cancer diagnosis and cancer care quality improvement work by supporting practices to improve their processes, achieve their targets & working with patients to help them ensure they have the right support at each stage of their journey.
You will work alongside social prescribing link workers to provide an all-encompassing approach to personalised care enabling people to navigate through the health & care system.
You will work with a diverse range of people from different cultural & social backgrounds. The ability to work confidently & effectively in a varied, and often challenging environment is essential.
Main duties of the job
CARE CO-ORDINATOR DUTIES
Arranging translation of letters for non-English speaking patients
Call & recalls for practice health campaigns
Fast track referrals chasing patients who havent attended
Tasks from GPs
Coordinating learning disability reviews & and supporting clinicians with adminstrative elements
First Contact Physio Appointments
Cancer Care searches and reviews
Undertaking clinical system searches
Mental Health Care Plans
Frequent flyers list
