Job summary
Based within Oakeswell Health Centre and part of Central Health Partnership you will work closely with the practice and PCN in delivering excellent healthcare to our patients. We reconise that looking after each other and team work, is key to excellent clinical care and our working lives. We are looking for a paramedic with Primary Care experience to join and compliment our workforce. The permanent postion is well suited to individuals who are enthusiastic about primary care and providing high quality support to the patients and wider clinical team. You will have excellent communication and interpersonal skills, with a good team approach to work. You will be dependable and have a strong work ethic, is mature and professional in your attitude. You will be self motivated and keen to develop new skills. You will be a member of HCPC and have a prescribing qualification and experience, as well as evidence of relevant CPD.
Main duties of the job
The following are core responsibilities of the PCN Paramedic
- You will act as a autonomous clinician, with support and guidance from the GP's.
- Assess, diagnose, plan, implement and evaluate interventions/treatments for patients with complex needs.
- Proactively identify, diagnose and manage treatment plans for patients at risk at developing a long term condition.(As appropriate)
- Diagnose and manage both acute & chronic conditions, intergrating both drug and non - drug based treatment methods into a management plan.
- Prescribe & Review medication for theraputic effectiveness, appropriate for the patient needs.
- Manage caseload of housebound chronic disease management patients.
- Telephone triage as appropriate, for clinics and home visits.
- Take part in clinics if and when needed.
- Undertake home visits, manage referrals and admit patients when required.
- Prioritise health problems and intervene appropriately to assess the patient in complex, urgent or emergency situations.
- Support patients to adopt health promotion strategies.
- Undertake, audits, appraisals, health promotion and teaching/mentoring staff.
- Implement and participate in vaccination programmes.

The job will be 32 hours per week over 4 days (Monday, Wednesday, Thursday & Friday) based at Oakeswell Health Centre, who is lead employer for this position.

- To co-ordinate telephone triage, taking clinical responsibilities of all patients who have requested an urgent appointment.
- The role will provide a specialist paramedic rescource for healthcare professionals and service users, working in collabaration with other members of the multi disciplinary team.
- To work within the surgery, as an autonomous, accountable paramedic, in the provision of a holistic approach for individuals including assessment, diagnosis and treatment to deliver quality patient services.
- The post holder will used advanced clinical skills to provide education to patients and service users, promoting selfcare and empower them to make informed choices about their treatment.
- To perform any other clinical and administrative duties where trained and determined safe to do so by the partners. This may include assisting with medical reports, safeguarding, and clinical support.
