Job summary

We are looking for an experienced and enthusiastic Practice Nurse to join our friendly and supportive team. The successful applicant will work alongside other clinicians, and in the local community, to provide the best care possible for our patients.

Main duties of the job

The main duties will be:

  • to undertake regular clinics within the Practice;
  • To be able to maintain our long-term condition register;
  • have a good knowledge and understanding of infection control;
  • to hold Practice Nurse clinics such as cervical screenings, childhood immunisations, travel vaccines;
  • to clinically support other members of our Clinical Team.

About us

Woodhouse Health Centre is a large GP practice in the south of Sheffield we currently have over 12,000 patients registered with us. The Practice has a large team of GPs, Nurses, Admin and Reception staff who are friendly, long-serving employees. The Practice is also a training practice for trainee GPs and Doctors. We also run our own Extended Access service which provides evening and weekend appointments for our local GP surgeries network.

Job description

Job responsibilities

The following are the core responsibilities of the Practice Nurse. There may be, on occasion, a requirement to carry out other tasks. This will be dependent upon factors such as workload and staffing levels:

Providing assessment, screening and treatment services and health education advice;

Provide nursing treatments to patients in participation with General Practitioners or independently to agreed protocols;

Provide general and specific health screenings to the Practice patients (within agreed protocols) with referral to General Practitioners as necessary;

Advise patients on general health care and minor ailments with referral to GPs as necessary;

Cytology and sexual health screening;

Assist in managing and monitoring Practice cervical cytology failsafe system;

Chronic Disease management (e.g. care of patients with Diabetes, CHD, COPD, Asthma);

Contraception advice;

Wound care / ulcer care / doppler;

Assisting in running INR Clinic near patient testing and dosing using INR Star;

Removal of sutures;

Travel vaccinations;

Asthma / Peak Flow Readings / Spirometry;

Venepuncture;

Hypertension Management;

Spirometry;

Routine/ childhood immunisations;

Urinalysis;

Blood pressure measurements;

Requesting basic pathology tests (e.g. urine culture, swabs);

Following agreed Infection Prevention and Control, and all clinical protocols.

Any other duties required within the Nursing Team role.

Qualifications

Essential

  • Registered Nurse with current NMC registration.
  • Significant experience in General Practice nursing, including chronic disease management.
  • Strong organisational and communication skills.

Desirable

  • Postgraduate diploma or degree (Chronic Disease Management).
  • Nurse prescriber.

  • Experience of working in a Primary Care environment.
