Healthier West Wirral are looking for An Advanced Clinical Practitioner who is passionate about Primary Care to join our well established multidisciplinary team. Your role will require you to participate in all aspects of patient care ensuring you maintain and expand on your advanced clinical skills.
Advanced practitioners can be
nurses, pharmacists, paramedics, physiotherapists, occupational therapists,
dietitians, or podiatrists. They are educated to masters level or equivalent,
with the advanced skills and knowledge to allow them to expand their scope of
practice to better meet the needs of the people they care for. Advanced
practitioners work at a level of advanced practice that pulls together the four
pillars of: clinical practice, leadership and management, education, and
research. Using their advanced skillset and knowledge, they can manage
undifferentiated cases and provide supervision across multiple professional
groups.
Main duties of the job
- Assess and triage patients, including same day triage, and as appropriate provide definitive treatment (including prescribing medications following policy, patient group directives, NICE (national) and local clinical guidelines and local care pathways) or make necessary referrals to other members of the primary care team.
- Provide care to patients at the point of entry to the health care system, including those with undifferentiated and undiagnosed health problems, manage those conditions, identify red flags and underlying serious pathology and take appropriate action.
- Use complex decision making to inform the diagnosis, investigation, complete management of episodes of care within a broad scope of practice.
- To provide continuing care to patients across the whole practice population.
- To provide health maintenance and health promotional services to the well, offering educational supportive setting for clients to explore their health problems.
- To provide psychosocial as well as physical care to the practice population.
- Actively take a personalised care approach and population centred care approach to enable shared decision making with the presenting person.
- Complete the relevant training in order to provide multi-professional clinical practice and CPD supervision to other roles within primary care, for example first contact practitioners and the personalised care roles.

Healthier West Wirral PCN supports a population of over 70,000 patients across 6 different Practices including Marine Lake Medical Practice, Heswall and Pensby Group Practice, Commonfield Road Surgery, Upton Group Practice, West Wirral Group Practice and Greasby Group Practice.
We currently recruit a team of over 30 practitioners including pharmacists, physician associates and paramedics.

Key duties
- To undertake chronic disease management (i.e., asthma, diabetes, hypertension, coronary heart disease), incorporating monitoring progress, response to treatment, reducing unnecessary hospital admissions and promoting the preventative aspects of clinical practice.
- Provide comprehensive family planning service including advice on emergency contraception.
- Initiate, perform and interpret ECGs, acting on results accordingly.
- Initiate laboratory requests and collect specimens as required. Interpret results and act accordingly.
- Manage commonly occurring minor illnesses/injuries and refer to other health professionals as appropriate.
- To recognise abnormalities and screen patients for disease risk factors and early signs of illness.
- Support other services provided within the general practice setting within agreed parameters.
Audit & Research
Essential
- Ensure that effective communications systems are set up, which include referral protocols, guidelines and policies for the management of patients in conjunction with other members of the PCN.
- Continuously evaluate and review activity and the effectiveness of care through audit.
- Participate in relevant research programmes and explore opportunities to undertake research in order to enhance the body of knowledge in a specialized field as appropriate.
- Disseminate research findings appropriately to other members of the PCN as well as the wider healthcare field.
- Play an active role in the Clinical Governance agenda at practice level and act as a resource to other practice members.
- Provide ongoing assessment of local health needs via profiling.
Management

- Ensure the maintenance of accurate records in line with practice policy and UK guidelines.
- Compile relevant statistics to meet the requirements of the PCN and Health Authority.
- Participate, together with other members of the PCN in service reviews.
- Initiate and maintain appropriate Clinical Supervision for the clinical team within the practice.
- Maintain own professional development in line with Post-Registration Education and Practice (PREP).
- Explore opportunities to network with other similar post-holders, to share ideas and provide professional support.
- Establish an effective stock control system.
- Attend clinical, managerial, PCN and other meetings as required.
Professional Development

- To continue professional development through a variety of strategies including attending study days, clinical placements, lectures and appropriate courses.
- Act as mentor to other clinicians where appropriate.
- Act as tutor to student and conversion course clinicians as appropriate.
- The postholder will participate in any training programme implemented by the PCN as part of this employment, such training to include:
- Participation in an annual individual performance review, including taking responsibility for maintaining a record of own personal and/or professional development,
- Taking responsibility for own development, learning and performance and demonstrating skills and activities to others who are undertaking similar work,
- Training may need to be undertaken outside of normal practice hours, and off site.
