An exciting opportunity has become available to join the Valentine Health Partnership. We are looking to recruit an enthusiastic, highly motivated and experienced Practice Nurse to join our General Practice Team who acts within their professional boundaries, supported and working alongside a team of general practice pharmacists and technicians.
The post holder is responsible for the delivery of practice nursing services care to the practice population. Supported by senior nurses within the practice, they will deliver care within the boundaries of their role, clinically the focus of the role is the delivery of evidence-based practice for patients with long term conditions and management of preventative nursing interventions to all patients. They will work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures, and providing nurse leadership as required.
Main duties of the job
The Practice Nurse is essential in providing comprehensive nursing care, focusing on patient assessment, treatment, and management within a primary care setting.
- The Practice Nurse is expected to deliver care to promote health and well-being of patients & supporting patients to adopt health promotion strategies to encourage live healthily & apply principles of self-care.
- Deliver individual treatment plans for patients with known long-term conditions. Referring to and working with other health as appropriate & in line with national and local policies.
- Prioritise health problems & intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care
- Provide information and advice on prescribed or over-the-counter medication on medication regimens, side effects and interactions
- Undertake procedures including BP management, wound management, ear irrigation, cytology and swabs, contraception advice, injections under PGD/PSD (e.g. B12/zoladex), peak flow,urinalysis, blood glucose monitoring
- Recognise, assess & refer patients presenting with mental health problems.
- Implement & participate in vaccination and immunisation programmes for both adults & children as per national & local programmes.
- Advise, support and, where appropriate, administer vaccinations for patients travelling abroad
- Review patients medication for therapeutic effectiveness & appropriate for patient needs in conjunction with practice policy
