Main duties of the job
We are seeking a motivated and experienced Practice Nurse to join our dedicated primary care team. The successful candidate will play a key role in the delivery of high-quality, patient-centred care and will be expected to actively contribute to the development of the nursing role within the practice.
You will be fully involved in both the clinical and administrative aspects of General Medical Services (GMS), including:

Management of long-term conditions and chronic disease patients

Delivery of QOF (Quality and Outcomes Framework) targets

Participation in enhanced services

Full attendance at practice meetings to support collaborative, multidisciplinary care
We are looking for someone with prior Primary Care experience, who brings excellent clinical competencies, as well as strong communication and interpersonal skills, ensuring safe and effective care for our patient population.

Northgate Medical Centre is located in the heart of Chester, we are a warm, welcoming, and patient-focused GP surgery dedicated to delivering the highest standard of care to our patients.
We operate from a fully modern, purpose-built facility, designed to support excellent clinical care in a comfortable and accessible environment. We currently have a list size of approximately 8,800 patients.
Our team consists of 3 GP Partners, 5 Salaried GPs, Practice Manager, 2 Deputy Practice Managers, 1 Advanced Nurse Practitioners, Nurse Manager, 2 Practice Nurses, Nurse Associate, Healthcare Assistant, GP Assistant and an extremely experienced Administration and Reception Team.

- Implement
and evaluate individual specialised treatment programs for chronic disease patients
- Respiratory Care including
Peak Flow Readings & Spirometry
- Diabetes Care Management
- Cardiovascular Disease and Hypertension
Management
- Cytology & Women's Health including Pill
Checks
- Immunisations for Adults and Children
- Would Care including Complex Dressings,
Compression & Doppler
- Travel Assessments and Vaccinations
- Ensure accurate records of all treatments and consultations are recorded on the clinical
system, using SNOMED codes and templates as appropriate
- Support patients in the use of their
prescribed medication or over the counter medication (within own scope of practice),
reviewing annually as required
- Support the practice to ensure targets are met for QOF, LES
& DES
- Collaborate with other agencies and services to
help patient's wellbeing
- Ordering and checking of stock and vaccinations
- Support and work together as required on any research initiatives with the Practice
- Visit housebound patients at home when required
- Contribute to the
education and training of students and participate in staff inductions to
include awareness of infection control
- Provide education
by attendance at courses and study days as considered relevant to your
professional on-going education
