Job responsibilities

Undertake GP Practice Nurse roles and responsibilities including:

• Chronic disease management
• Immunisations and vaccinations
• Cervical screening
• Contraceptive advice
• Injections and treatments as prescribed
• Lifestyle advice
• Health promotion
• Maintaining accurate patient records
• Clinical audits
• Team meetings
• Referring patients as appropriate
• Attending training and development as required
• Clinical supervision and appraisal

Person Specification

Qualifications

Essential

• Registered Nurse (RN) qualification and current registration with the Nursing and Midwifery Council (NMC).
• Knowledge of chronic disease management and health promotion.
• Excellent communication and interpersonal skills, with a patient-cantered approach.
• Strong organisational skills, with the ability to manage multiple tasks effectively.
• Ability to work independently and as part of a multidisciplinary team.
• Commitment to continuous professional development and learning.

Desirable

• Previous experience in primary care or community health settings
