We have an exciting opportunity for a motivated and forward-thinking Clinical Pharmacist to join Greens Norton & Weedon Medical Practice (GNWMP), working in-house across two sites as part of a well-established and supportive multidisciplinary team.

This role is ideal for an experienced pharmacist within general practice, who is passionate about delivering high-quality, patient-centred care and driving improvements in medicines optimisation.

You will play a key role in shaping and delivering modern primary care services, helping patients to stay well, use medicines safely, and avoid unnecessary hospital admissions.

Essential requirements:

  • Completion of the CPPE Primary Care Pharmacy Education Pathway
  • Full driving licence

Main duties of the job

Main Duties of the Role:

This is a varied and rewarding patient-facing role where you will have a real impact on patient outcomes and practice efficiency.

Working across two sites, you will:

  • Deliver structured medication reviews, particularly for patients with complex needs and polypharmacy
  • Lead on medicines optimisation, safety, and cost-effective prescribing
  • Manage repeat prescribing systems and acute medication requests
  • Carry out medicines reconciliation following hospital discharge and transfers of care
  • Run clinics (face-to-face, telephone, and remote where appropriate)
  • Support care homes and undertake domiciliary visits where required
  • Provide expert clinical advice to GPs, nurses, and the wider practice team
  • Respond to medication queries from patients and healthcare professionals
  • Support delivery of QOF, IIF, and other service targets
  • Contribute to audits, quality improvement, and service development
  • Implement national guidance, safety alerts, and local prescribing initiatives

Your work will directly contribute to reducing avoidable hospital admissions, improving patient safety, and enhancing the overall quality of care.

Role Overview: The Clinical Pharmacist works within their professional boundaries, alongside a team of pharmacists in our two site practiced to improve patients health outcomes and the efficiency of the primary care team by providing direct, timely medicines expertise, including face-to-face contact with patients.

The post-holder will be an integral part of the general practice team and will optimise medication use, support clinical staff, and act as a conduit of patient medicine information between primary and secondary care.

Core ResponsibilitiesPatient-Facing Clinical Work
  • Work as part of a multi-disciplinary team to clinically assess and treat patients using expert knowledge of medicines.
  • Prescribe (or complete training to become an independent prescriber) alongside the general practice team.
  • Manage care for patients with chronic conditions, complex polypharmacy, frailty, COPD, asthma, learning disabilities, or autism, including STOMP (Stop Over-Medication Programme).
  • Conduct structured medication reviews to optimise adherence, reduce waste, and support self-care.
  • Deliver long-term condition clinics, home visits, and pharmaceutical consultations as part of the MDT.
  • Review medications for newly registered patients and high-risk patients (e.g., anticoagulants, anticonvulsants, DMARDs).
  • Actively signpost patients to appropriate healthcare professionals.
  • Support patients with drug and alcohol dependencies and complex mental health or end-of-life care needs.
Medicines Optimisation & Leadership
  • Provide specialist advice on medicines to patients, staff, and the wider healthcare team.
  • Working as part of our clinical pharmacy team on person-centred medicines optimisation, antimicrobial stewardship, and quality improvement initiatives.
  • Support the integration of general practice with community and hospital pharmacy services.
  • Manage repeat prescribing and medication review systems, including safe storage, rotation, and disposal of drugs.
  • Promote cost-effective prescribing and adherence to NICE, CQC, and clinical guidance.
  • Participate in clinical audits and service redesign to improve prescribing effectiveness.
Administration & Governance
  • Maintain accurate clinical records in line with legislation.
  • Participate in enhanced services, public health campaigns, and clinical governance.
  • Contribute to complaints management, clinical incident reviews, and organisational learning.
  • Undertake mandatory training, induction, and formal appraisals at least annually.
