This is a unique role which will place you as a key contact to support patients recently diagnosed with cancer and also a key contact for patients and their carers for patients that are on the end of life/GSF pathway.

Main duties of the job

Working as a part of PCN clinical team, you would help our patients to understand their condition and what matters to them and then match the support available locally. Crucially, you will also work with the practices in ourPCN to support them with Early Cancer Diagnosis DES. This will mean that as part of the role you would take part in regular MDT meetings, increase early cancer diagnosis rates by promoting national screening programmes, organising educational events and case reviews.

As well as being efficient you must also display empathy for both our patients and their carers, this will come with its challenges as you will often be a cancer patients first point of contact .

From first point of contact to main point of contact, this unique role will allow you to develop a strong relationship with cancer patients and their carers.

Job responsibilities

Primary responsibilities

There may be, on occasion, a requirement to carry out other tasks.

a) Ensure that all patients are signposted to, or receive information on their referral including why they are being referred, the importance of attending appointments and where they can access further support

b) Be available to patients with cancer, or their relatives, to ask questions and deal with problems that arise, linking in or signposting to services such as the hospital team, district nursing or Macmillan services, benefits agency as appropriate.

c) Review patients and follow up each patient for a period after three months of a cancer diagnosis, covering topics such as benefits, support groups, offering support to relatives

d) Arrange a Cancer Care Review six months after the initial diagnosis with a Practice Nurse

e) Be a point of contact for survivors of cancer, or bereaved relatives, who need support, signpost to support groups available locally or nationally as appropriate.

f) Support the Practice in your PCN to improving local uptake of National Cancer Screening Programmes, understand the practices data for uptake on screening and understand how the practices can carry out the screening

g) Support the Practices in your PCN in conducting peer to peer learning events that look at data and trends in diagnosis across the PCN, including cases where patients presented repeatedly before referral and late diagnoses

h) Support with the education for practice teams and patients and co-ordinate screening promotion events with the Practices across the PCN

i) Link in with and build relationships with the wider PCN Team, Social Prescribers, Pharmacist, Health and Wellbeing Coaches and other clinical/non-clinical partners involved in patients care

j) Holistically bring together all of a persons identified care and support needs, and explore options to help them achieve their needs

k) Help people to manage their needs through answering queries, making and managing appointments, and ensuring that people have good quality written or verbal information to help them make choices about their care

l) Overall responsibility for arranging MDT/GSF Meetings, arranging the meetings across the PCN, managing the meeting agenda items; ensuring that all new referrals are identified, and information circulated to team members in advance of the meeting

M) Take minutes of MDT/GSF meetings and disseminate; chase progress against actions identified in these meetings and ensure follow up where necessary
