Job summary

This is a great opportunity to join and help to continue to develop an established small team delivering high quality care to patients within 3 care homes within the Burmantofts, Harehills and Richmond Hill area. We work toward meeting the principles of the Enhanced Health in Care Homes DES.

The team consists of x2 ACPs with administrative support and currently daily input from Nursing Associates - although we are constantly reviewing the model to find the most effective and efficient format for delivery...and you would have input in to how this model develops moving forward.

Burmantofts, Harehills and Richmond Hill PCN has centralised delivery of care for patients within a care home setting and these patients are registered at Bellbrooke Surgery so even though the service is PCN funded we hope you have the benefit of being part of the Bellbrooke team and want to avoid any feeling of isolation sometimes associated with PCN working.

Bellbrooke Surgery GPs aim to provide comprehensive support on both a daily basis and toward ongoing professional development. Although we aim to have x2 WTE ACPs within the Care Homes team we believe that dedicated time required is less than this and allows both clinicians to undertake additional work...perhaps undertaking acute appointments or visits or managing a small portfolio of frail/housebound patients. The intention is you will help drive how this role looks to best meet your career aspirations and development needs

Main duties of the job

Support people entering, or already resident in the PCNs Aligned Care Home/s.

Support the PCN pharmacists to produce Structured Medication Reviews and Medicines Optimisation.

Work with all relevant partners within a multidisciplinary team (MDT) to deliver Enhanced Health in Care Homes service requirements.

Deliver a weekly home round for patients living in the PCNs Aligned Care Home(s). Prioritise residents for review according to need based on clinical judgement and care home advice. Use digital technology to support the weekly home round and facilitate medical input as appropriate.

Aim for a personalised care and support plan to be developed and agreed with each new patient within seven working days of admission to the home and within seven working days of readmission following a hospital episode (unless there is good reason for a different timescale). Develop plans with the patient and/or their carer and base plans on the principles and domains of a Comprehensive Geriatric Assessment including assessment of the physical, psychological, functional, social and environmental needs of the patient including end of life care needs where appropriate.

Make all reasonable efforts to support delivery of the plan.

Identify and/or engage in locally organised shared learning opportunities as appropriate and as capacity allows.

Support with a patients discharge from hospital and transfers of care between settings, including giving due regard to NICE Guidelines.

About us

Bellbrooke Surgery are a forward thinking and well regarded GP practice based in the centre of Harehills in a modern, air conditioned building with parking available on site. We have around 14700 patient in total.

We have 11GPs and a strong team of ACPs, nurses, nurse associates, health care assistants and reception/administrative staff. We are a training practice and provide the level of care and support you would expect in a positive environment.

The care homes team work from their own office within the practice and have the benefit of significant autonomy but with support always available. All the homes we manage are within a 2 mile radius of the practice and we have good relationships with each home. We look after approximately 100 patients within a care home setting.

Bellbrooke Surgery are members of Burmantofts, Harehills and Richmond Hill Primary Care Network (PCN), one of the largest PCN's in the city with 12 member practices looking after around 87000 patients.

We have other PCN services delivered on site including First Contact Physio, Social Prescribing, Pharmacy services, dedicated Mental Health services and staff supporting other ad hoc projects.

Job Summary

ACPs can be found practising across a range of fields, such as nursing, pharmacy, emergency medical services (paramedics) and adult and children's therapies. You will be:

  • a clinical professional who has developed their skills and theoretical knowledge to a high standard.
  • an experienced and educated member of the care team who can diagnose and treat health care needs or refer patients to an appropriate specialist if needed.
  • Trusted and empowered to make high-level clinical decisions and will often have your own caseload in this instance, primarily Care Home Patients

As an ACCP, you will be expected to perform many of the tasks and take on a similar level of responsibility to trainee medical staff who have a formal regulator in the GMC. These duties may be performed with direct, indirect, or distant supervision by a GP Lead or similar,

depending on the task and situation.

You will have excellent communication skills, a compassionate and friendly nature who can operate in a busy environment with a range of people and professions, as you will be working our clinical and administration teams.

Job Responsibilities

CRITICAL CARE

  • Undertaking comprehensive clinical assessments of patients
  • Requesting and performing tests to diagnose a patient's condition.
  • Managing episodes of patient care
  • Leading and supporting within a team of different healthcare professionals and staff
  • Using different sources of information to assess and manage the care of a critically ill patient.
  • Undertaking clinical audits

PATIENT ASSESSMENT

  • Evaluating and/or collecting information from physiological assessments, patient history, and diagnostic data to identify relevant problems:
    • Advanced levels of patient assessment
    • Diagnostics and decision-making
    • Advanced patient interventions
    • Advanced non-medical prescribing
  • Using clinical decision making to initiate and adjust appropriate management/treatments according to agreed protocols and clinical judgement.
  • Evaluating patients progress against treatment plans through interpretation of information from test results and clinical findings.

PATIENT SAFETY AND QUALITY IMPROVEMENT

  • Participate and lead in the collection and evaluation of clinical audit data.

  • Delegates appropriately and encourages staff to develop and advance their skills.
  • Ensure the clinical team are kept informed of the progress of the role and provide opportunities for suggestions and comments to improve care delivery.
