Job summary

We are looking for a forward thinking and highly motivated GP to complement our friendly team of 11 GP's. Providing high quality care to a growing list of nearly 20,000 patients, with a varied demographic, we have an opportunity for a GP interested in a 6-8 session salaried role and with a passion to deliver and drive high quality patient centred care in our forward-thinking training practice.

Based in the excellent location of aviation renowned Farnborough, we have a fabulous purpose designed building on the Hampshire/Surrey border. Within easy reach of the M3 and M25, we are co-located with community nursing, an integrated care team, community midwifery and paramedic home visiting team with regular patient centred and co-ordinated MDT meetings.

As a well-respected GMS training practice, we aim to provide good access for patients with urgent problems but also provide continuity for those with more complex needs.

Main duties of the job

The post-holder will manage a caseload and deal with a wide range of health needs in a primary care setting, ensuring the highest standards of care for all registered and temporary patients.

As GPs, we work alongside a large and highly skilled team of Nurse Prescribers, Paramedic Practitioners, Physician Associates, Practice Nurses, a Mental Health Practitioner, Clinical Pharmacist, Pharmacy Technician and Physiotherapists. An organised and dedicated admin team are employed to support us all.

We outsource our workflow and have a full time GP Assistant to help minimise admin for GPs and we have strong links and input with the active Farnborough Primary Care Network (PCN).

The locality led home-visiting team of paramedics do > 95% of our daily visits, leaving us to concentrate on end-of-life care.

Job responsibilities

Clinical responsibilities:

  • In accordance with the practice timetable, as agreed, the post-holder will make him/her-self available to undertake a variety of duties including surgery consultations, telephone consultations and queries, visiting patients at home, checking and signing repeat prescriptions and dealing with queries, paperwork and correspondence in a timely fashion
  • Making professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the organisation
  • Assessing the health care needs of patients with undifferentiated and undiagnosed problems
  • Screening patients for disease risk factors and early signs of illness
  • In consultation with patients and in line with current practice disease management protocols, developing health care plans for health
  • Providing counselling and health education
  • Admitting or discharging patients to and from the caseload and referring to other care providers as appropriate
  • Recording clear and contemporaneous consultation notes to agreed standards
  • Collecting data for audit purposes
  • Compiling and issuing computer-generated acute and repeat prescriptions
