Are you a registered Nurse Practitioner looking to join a fantastic and diverse team?
We are looking for a NP to complement our friendly team of nurses and GP's providing care to a growing list of nearly 20,000 patients.
We have developed a model which separates out on the day from 'routine' care. We aim to provide good access for patients with urgent problems but also provide continuity for those with more complex needs.
Main duties of the job
To be responsible for the delivery of evidence-based nursing services, working as part of the practice multi-disciplinary team, delivering care within their scope of practice to the entitled patient population. Nurse Practitioners have the freedom and authority to act and make autonomous decisions in the assessment, diagnosis, and treatment of patients.
The NP provides an opportunity for patients to avoid the need to be referred to a GP, thereby enabling them to receive timely care and negate unnecessary delay in receiving treatment.Working autonomously, the NP will be responsible for several clinical areas such as health promotion, chronic disease management, health prevention, minor illness, as well as supporting the management team in the reviewing of clinical policy and procedure. The NP will be required to work in our urgent care department along side running chronic disease clinics.
NPs in primary care can develop close, long-term relationships with their patients and work in partnership with them to achieve optimum health. They are autonomous in making decisions based on assessment, diagnosis, and interpretation of test results.
They can independently prescribe appropriate medication, evaluate, or refer to other specialists if necessary.

integrated care team, community midwifery and paramedic home visiting team.
Although the largest practice in Farnborough, we are a

are the core responsibilities of the Nurse Practitioner. There may be, on
occasion, a requirement to carry out other tasks; this will be dependent upon

- Develop,
implement, and embed health promotion and wellbeing programmes
- Manage
patients presenting with a range of acute and chronic medical conditions,
providing subject matter expert advice
- Implement
and evaluate individual specialised treatment plans for chronic disease
patients

manage and support patients at risk of developing long-term conditions,
preventing adverse effects to the patients health
