Job summary

Care Coordinators are a key contact point for patients. They are seen as pivotal to our planned work and are evolving roles in support of staff and practices in delivery of effective care and services. At Market Weighton, you will be the link between administrative and clinical teams working on the practices total triage system using Accurx and SystmOne software. You will also support on other administrative work to support the clinical care of patients.

Main duties of the job

The successful Care Coordinator candidate will proactively work with people, including the frail/elderly and those with long-term health conditions, and will coordinate and navigate care and support for them, including making referrals where appropriate

The successful candidates will have previously worked in GP practice, other health, social care or support roles in direct contact with people, families or carers, paid or unpaid. They will demonstrate an understanding of the Personalised Care Approach; the needs of older people/adults with disabilities/long terms conditions, particularly in relation to promoting their independence. They will have a good understanding of confidentiality and the importance of Data Protection.

Experience of collating information and using tools to demonstrate the impact of services is required; knowledge of the SystmOne clinical system would be a real advantage.

This role offers competitive terms and conditions including the NHS Pension. The successful candidates will be required to work across a variety of locations.

About us

Harthill Primary Care Network (PCN) is a network of six NHS GP Practices within the Haltemprice and Market Weighton areas of the East Riding of Yorkshire which includes practices in Cottingham, Willerby, Anlaby, Hessle, North Ferriby and Market Weighton. We are one of seven PCNs across the East Riding and cover a registered population of over 34,000 patients.

Job responsibilities

Patient and GP Practice Support Work

To support the practice team with a range of duties which will change from time to time.

Monitoring and Processing of Single Point of Online Requests into the practice via Accrux

As part of the practice triage team, log in, monitor and process requests from patients via Accurx and the SystmOne Clinical Record System.

Liaise with the practice team to ensure that all incoming requests are dealt with appropriately within the target response time.

Use Teams and messaging systems to communicate with colleagues appropriately as needed.

Emails

To consistently monitor all emails and action any admin related queries.

To attend the monthly multi-disciplinary meeting following the CPA (on rotation).

To monitor and action all tasks within your personal box and team group task box on SystmOne.

Additional Clinical and Patient Support which may include the below, the examples are not exhaustive:

Spirometry and Feno Appointments

To monitor the Spirometry and Feno waiting list, making appointments following the protocol and informing the Rota Coordinator if appointments wont be filled.

Learning Disability Reviews

Ensure all patients with a Learning Disability have an annual review using the recall system.

Chronic Disease Reviews

Support the contracts team with booking any outstanding chronic disease reviews.

Overdue Cervical Smears

Identify patients overdue cervical screening and follow contact these patients as per the protocol.

Overdue Bowel Screening

To contact the patients who have not completed a FIT test to encourage uptake.

Blood Pressure Protocol

To monitor the BP loan machine waiting lists for all sites. Actively loaning out machines and chasing when not back within scheduled timeframe.

To manage both the Floreys and scanned on results and action as per protocol.

To chase any non-responders.

Helping patients with any queries they may have and supporting them in making appointments.

Contacting the patient on behalf of the GPs guidance regarding medication changes.

Baby Vaccines

Using the data provided from the child health team at Humber checking our clinical system to see who overdue vaccine is. Updating the document for the team at Humber and actively contacting those overdue patients to make an appointment.

Above are the main clinical workflows currently aligned to the team however throughout the year more may be identified which could require the support from the team.

Flu and Covid Vaccine Workload:

To provide support with organising clinics and booking appointments for the Covid and Flu Vaccination Programme as and when required.

Coordinating the role out to housebound and care home patients including route planning.

Ensuring all care home consent forms are correctly completed, patients list are up to date along with supporting the team to administer vaccines.

Help in clinics when needed.

To support with inputting vaccine data into SystmOne, ensuring the data quality of vaccines is accurate to maximise vaccine claims and eliminate any errors.

Ensuring all vaccinators and admin have completed all required training before each phase.

Report accurate weekly stocktakes and checking to make sure the vaccines are stored safely and correctly.

To support with moving vaccines between sites whilst ensuring the cold chain is maintained.

Other

To support the practices with rearranging clinics at short notice due to staff absence.

To support the Pharmacy team with any audit and review work as and when required.

Management of workload and personal development

Act on own initiative in prioritising and managing workload, in respect of clinical, managerial, and administrative areas, as necessary.

Maintain own personal and professional development as agreed at your annual review.

Other

To support the Service Manager and Management team with administrative duties. This will be ad hoc work.
