Job summary

This Care Coordinator role at LSMP and TLS PCN will be involved in a wide range of work to improve the outcomes of patients who need additional support in navigating the health and care system. This will include working directly with a diverse patient list, coordinating with a wider multidisciplinary team to tackle health inequalities, and engaging with patients and colleagues around the benefits of personalised care.

Interviews will be held in person on Wednesday 20th December 2023.

Main duties of the job

Support the patient journey directly by:

  • Developing personalised care and support plans to help manage patients' needs and improve their health outcomes- Working with patients, their families, and their wider support networks to improve the understanding of the patients' conditions
  • Providing coordination and navigation across the health and care system for patients, through answering patient queries and making and managing appointment
  • Assisting people to access personal health budgets and benefits where appropriate
  • Measuring the impact of your work through patient reporting to identify areas where your role can be developed to improve outcomes

This is a highly autonomous role that would suit a candidate who likes working under their own initiative. There is a requirement to fulfil project work, administrative responsibilities, and clinical work with patients. Each days workload can be very different, and the successful candidate will be confident in managing their own varied workload, working independently to organise their time to complete daily, weekly and monthly workload completion requirements.
