Job summary
Leeds Student Medical Practice and The Light Surgery Primary Care
Network (LSMP and TLS PCN) are looking to recruit a PCN Social Prescriber. This
is a newly created collaborative role between the PCN and Yorkshire MESMAC. Our
PCN is extremely diverse with patients mainly being students or young
professionals. We want the most resilient people to join our ever-expanding
team.
The role is extremely varied, with the successful candidate providing social prescriber support to our patient population, in particular the LGBTQIA+ community. This is a role that supports patients to address and tackle issues that are having an adverse effect on their mental and physical health, or social challenges such as isolation.
This role is a 12 month fixed term with potential to extend dependant on the success of the role.

Main duties of the job
The successful applicant will be required to work on their own
initiative as well as being a good team player. You will need to be a good
listener, empathetic, and understand the patient needs. You will also have
outstanding interpersonal skills and be able to work with patients to identify
wider health, wellbeing, and social interventions. An interest in health and
wellbeing is essential and we are willing to provide upskilling training for
the right applicant.

PCN Overview:
LSMP and TLS form our Primary Care Network, which works
collaboratively to ensure high quality, equitable, and sustainable care. We
work together to represent over 57,000 service users and strive to provide
outstanding support. Our work remains rooted in the local communities that we
serve which is predominately student and young adult populations.
LSMP currently holds the Gold Pride in Practice award-this has been in
place since 2019 and LSMP are looking to maintain this while building on this
foundation.
Yorkshire MESMAC overview:
Yorkshire
MESMAC is one of the oldest and largest sexual health organisations in the
country. We offer a range of services to various communities across Yorkshire,
including men who have sex with men, people of colour and other marginalised
races, people misusing drugs, sex workers and LGBTQIA+ young people and adults.
We offer a
range of mental health and wellbeing services, including counselling, 1-2-1 support,
group sessions and training.

Yorkshire MESMAC to deliver individualised, tailored support to improve the
wellbeing of our LGBT+ registered service users. This will be via holistic
assessments and integrated care plans to improve the health and well-being for LGBT+
individuals with a longer-term outcome of optimising the number of clinical
/medical interventions required.
The successful
candidate will be kind, empathetic and trustworthy to help put service users at
ease. They will be knowledgeable in the field of personalised holistic care and
will ideally understand the options available within the healthcare system to optimise
service user care. They will be passionate about reducing health inequalities,
and understanding of the challenges that impact good service user care. They
will be motivated to develop themselves and their role to meet the requirements
of the PCN.
