Job summary

Forest Medical are looking for an experience Practice Nurse who currently holds a active registration with the NMC, to join our large practice covering two sites, one in the centre of Mansfield and our second site in the centre of the Oak tree Lane Estate.

Preferably with wound care experience, chronic disease management, cytology, and immunisations skills.

Must have at least 6 months practice nurse experience.

Additional training will be offered to the right candidate in line with practice need.

  1. Forest Medical is a large GP surgery based in the heart of Mansfield, with a every growing list size currently in the region of 18,000 patients.
  2. The surgery is a very busy and fast paced, but team work is at the centre of our practice.
  3. The nursing team currently consists of 4 practice nurses and 2 HCAP, 3 HCA and 2 Phlebotomists.

Job description

Job responsibilities

To provide nursing care and long-term condition management to the practice population according to agreed protocols.

To identify, plan and develop the health promotion activities within the practice population in line with Quality and Outcomes Framework and the Health of the Nation targets.

To carry continuing responsibility for the assessment of nursing care needs, the development, implementation, and evaluation of performances of care, including screening and patient education and preventative care.

MAJOR DUTIES AND RESPONSIBILITIES:

1. To provide assessment, screening treatment services and health education advice.

(i) Organise and co-ordinate the provision of nursing services for the practice to include the delegation of duties to other practice staff as appropriate and cover across both sites.

(ii) Provide nursing treatments to patients in participation with General Practitioners or independently agreed protocols.

(iii) To carry out aseptic dressing techniques, ear syringing and other procedures subject to the necessary training required (if any).

(iv) To carry out extended role procedures, e.g. vaccination and immunisation, venepuncture, cervical smears, family planning, following certification of competence.

(v) Manage and monitor general and specific health screenings to the practice patients, within greed protocols, targeting specific at risk groups identified within the area, with referral to General Practitioners as necessary.

(vi) To participate in the practices asthma and diabetic clinics (as per protocols and rota), and to ensure efficient call/recall of patient groups (As per training and competency).

(vii) To undertake the screening of the elderly and housebound assessments within the practice, undertaking home visiting and liaising with other agencies where appropriate.

(viii) To participate within agreed protocols in any other activities e.g. travel clinics.

(ix) To participate in the practices annual flu immunisation campaign.

(x) To undertake and facilitate additional service level agreements agreed between the practice and primary care trust. For example new screening programmes and or vaccination programmes.

2. Treatment Room Supplies and Equipment and Procedures

(i) To participate in the efficient ordering and storage of consumables used within the practice and maintain in good order all equipment

(ii) Assist the General Practitioners with medical procedures as agreed.

(iii) Perform ECGs as and when reasonably requested.

(iv) To undertake the collection of pathological specimens including intravenous blood samples, swabs, smears etc. Perform any investigatory procedures requested by General Practitioners.

3. Administrative and Professional Responsibilities

Participate in the administrative and responsibilities of the practice team.

(i) Ensure accurate notes of all consultations and treatments are recorded in the patients notes (or otherwise as agreed).

(ii) Ensure accurate completion of all necessary documentations associated with patient health care and registration with practice manual and/or computerised.

(iii) To be familiar with the practice computer system

(iv) Attend and participate in practice staff and primary health care team meetings and to liaise with them, as required.

(v) Assist in the formulation of practice philosophy, strategy and policy.

(vi) Develop nursing policy and protocols in collaboration with the nursing team and General Practitioners to contribute to the formulation of practice philosophy and strategy.

4. Research Projects and Audit

(i) Co-operate and participate as required in any research projects within the practice..

(ii) Participate in the training of students, of all disciplines and the induction of all members of the practice staff with special responsibilities for nursing staff.

5. Education and Professional Development

(i) To attend training courses and study days as deemed useful or necessary for professional development.

(ii) Responsible for maintaining an up-to-date knowledge of professional nursing matters (e.g. trends and procedures), identifying training needs as appropriate.

(iii) Work within the NMC professional guidelines and to be aware of current legislative agreements in relation to his/her work.

(iv) Participate in education and training of students of all disciplines and the induction of all members of practice staff with special responsibility for nursing staff, as and when agreed.

6. Liaison

Co-ordinate and monitor effective liaison with other agencies and staff concerned with patient care and with all other disciplines within the practice, with appropriate regard to confidentiality.

7. Infection Control Responsibilities

(i) Have awareness of local infection control responsibilities/Audits and personal and practice responsibility

(ii) Educate individuals and groups about the risk, prevention, transmission, and control of infection, disease-specific care, appropriate precautions, and appropriate assessments

(III) Establish accepted standards and develop, implement, monitor and revise infection control policies and procedures to assure compliance with the standards

(IV) Investigate, manage and conduct surveillance of suspected and confirmed outbreaks of infection
