The role reports to the practice manager.

As an ANP, you will manage a broad clinical caseload across acute, urgent, and long-term conditions. You will be involved in chronic disease management, on-the-day care, and collaborative decision-making alongside our GPs, nurses, HCAs, and wider multi-professional team.

  • Strong clinical reasoning, communication, and decision-making skills

  • Experience in primary or urgent care

  • A reflective, team-oriented professional with emotional intelligence and a commitment to high standards

Main duties of the job

This is a varied and autonomous clinical role suited to a highly skilled Advanced Nurse Practitioner. You will assess, diagnose, treat, and manage patients with a wide range of acute and chronic conditions, using expert clinical reasoning and evidence-based decision-making. You will provide personalised care in partnership with patients and families, contribute to chronic disease management, and may prescribe medications and treatments where qualified.

You will be a key part of the multidisciplinary team, working within your professional code and scope of practice, exercising judgement, managing risk, and recognising the limits of your competence. You will reflect critically on your practice, support safeguarding, lead service improvements, and help shape high-quality patient care.

The role also includes mentoring, peer support, and contributing to governance, QOF, audit, and significant event reviews.

If you're passionate about delivering excellent care and supporting innovation in primary care, we welcome your application.
