Job summary

National Funding has been secured to develop more integrated ways of working for children and young peoples services in Chard. This presents an exciting opportunity to be part of an innovative 12-month pilot project, with the potential to be a flagship model in the country. The practitioner will work closely with health, social care and VCSE system colleagues to contribute to a same day urgent care paediatric service within the primary care team, offering high-quality primary care treatment close to patients homes within a timely manner.

This additional capacity will be resourced for one year and serve patients aged 18 years and under registered at either of the two surgeries in Chard.

We are seeking an enthusiastic and committed Advanced Nurse Practitioner (or role of equivalent clinical competency) to join our established and highly motivated team at Ariel Healthcare and Springmead Surgery. This post available would work 37.5 hours a week, Monday to Friday.

Salary will be dependent on experience. Hours and working patterns to be discussed at interview, but this is a full-time position.

Main duties of the job

The post holder will be an Advanced Nurse Practitioner (or practitioner with equivalent clinical competency) with experience/training in Paediatrics. They will be responsible for independent clinical assessment and examination, diagnosis, planning of treatment, prescribing, health promotion and evaluation of patient care. Supported by the duty doctor, they will have the ability to work within Primary Care as an integral part of the team, working independently and using an advanced level of professional accountability, autonomy, and judgement.

The post holder will support parents and carers to follow a single pathway to access this same day urgent care provision in their community, referring onto secondary care as required.

This is an innovative position with opportunities for the practitioner to lead on paediatric training and development of staff within the practice, seeking opportunities to develop pathways and cascade learning across the system.

The post-holder will work closely with community, acute, and urgent care colleagues to increase health literacy within the community, developing strategies for health promotion and increasing parent/carers understanding of when and how to self-manage minor childhood conditions, aided by promotion of the Paediatric HandiApp.

The post holder will work collaboratively as part of a multi-disciplinary team, undertaking quality improvement of policies and standardisation of care within the practices.

This is an innovative role and we are looking for an Advanced Nurse Practitioner who has specialist skills in paediatrics (or role of equivalent clinical competency) to provide additional capacity to the provision of a same day primary care service for under 18s, offering face to face, telephone or video appointments.

Advanced clinical practice is delivered by experienced, registered health and care practitioners. It is a level of practice characterised by a high degree of autonomy and complex decision making. This is underpinned by a masters level award or equivalent that encompasses the four pillars of clinical practice, leadership and management, education, and research, with demonstration of core capabilities and area specific clinical competence. Advanced clinical practice embodies the ability to manage clinical care in partnership with individuals, families, and carers. It includes the analysis and synthesis of complex problems across a range of settings, enabling innovative solutions to enhance peoples experience and improve outcomes.

  • To be responsible for the rapid assessment, diagnostics, and treatment of individuals within the primary care service using an advanced level of professional accountability, autonomy, and judgement, underpinned by Masters level theory and experience. This includes highly complex decision-making across a broad range of differentiated and undifferentiated individual presentations and complex situations, synthesising information from multiple sources to make appropriate, evidence-based judgements and/or diagnoses.
  • To plan and manage complete episodes of care. This will be working in collaboration with senior medical colleagues and others when required.
  • Delegating and referring as appropriate, to ensure timely, effective management to optimise health outcomes, in line with the evidence base and nationally recognised best practice.
  • To play a key operational role and maintain a pro-active approach to managing, updating and leading clinical pathways.
  • To promote and implement service and policy development and redesign, impacting beyond own area and other disciplines, informed by current best practice and evidence-base, in accordance with values-based care.
  • To ensure high quality learning environments for all disciplines within a context of multi-professional education and training that is responsive to future changes and challenges.
